42-02 · Technical assistance was provided that the child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program

03-01 · Technical assistance was provided that The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one child care personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times. Director and another staff came in to the facility at 2:15PM. Per director, she was out getting food and had called teacher to help her, then they got caught in the rain. At time of arrival, FSC knocked on the facility's door and a staff opened and stated director and one staff was not in. FSC explained that inspection still need to be conducted and staff said she would call director. Ratio at Time of Arrival: Infants and 1-Year Old - 1:5 2-Years Old and 3-Years Old - 1:11 4-Years Old - 5 children being supervised by the teachers on-site
