During the inspection the Family Service Counselor observed that one student Health Examination or equivalent health statement was not acceptable in that: [ It was not current (over two years old)]. Technical support that was given to the provider stated that the Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. This violation was resolved in the presence of the family service counselor during the routine inspection
