    During the commencement of investigation LS observed the incident report form was incomplete, in that it was missing the name, date, signature and time when the form was acknowledged by parent. Provider was able to correct at the time of inspection by having parent completed the missing areas and a copy was given to parent

    FSC was reviewing children files and noticed that five children distracted adult brochures haven't been done in the past 1-2 years. TA was given to provider referencing the CCF Handbook. 7.3 Enrollment Information The facility operator shall obtain enrollment information from the childs custodial parent or legal guardian prior to accepting a child in care. This information shall be documented on CF-FSP Form 5219, Child Care Application for Enrollment, which is incorporated by reference in 65C-22.001(7)(f), F.A.C., or an equivalent form that contains all the information required by the Department on CF-FSP Form 5219. CF-FSP Form 5219 may be obtained from the licensing authority or on the Departments website at www.myflfamilies.com/childcare. C. There shall be signed statements from the custodial parents or legal guardian that the childcare facility has provided them with the following information: 5. Annually, in the months of April and September, the child care facility must provide parents/guardians of the children enrolled in the facility with information regarding the potential for distracted adults to fail to drop off a child at the facility and instead leave them in the adults vehicle upon arrival at the adults destination.CF/PI 175- 12, brochure, which is incorporated by reference in 65C-22.001(7)(x), F.A.C. and may be obtained from the Departments website at www.myflfamilies.com

    1 of 18 personnel files was observed by FSC to not have completed the 40hr introductory child care training requirement. Technical assistance was given to provider per CCF handbook "Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. Part I Courses (30 Hours) Child care personnel must complete all of the following: ? Child Care Facility Rules and Regulations ? Health, Safety and Nutrition ? Identifying and Reporting Child Abuse and Neglect ? Child Growth and Development ? Behavioral Observation and Screening B. Part II Courses (10 Hours) Child care personnel must also complete 10 hours of the following Part II courses: ? Special Needs Appropriate Practices (10 hours), or ?
