License Specialist observed that there was no documentation to show that during the facility's licensure year, fire drills utilizing the approved alarm system were not conducted monthly since the last Routine Inspection was conducted on 10/24/2024. Fire Drill for the month of January will also be required by the time of re-inspection due by 2/12/2025. Technical Assistance was given to the Provider as per the School Age Child Care Facility Handbook that: ( During the facilitys licensure year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. *** The operator must maintain and post in an easily seen location a written record of emergency preparedness drills showing the type of drill, date conducted, number of children and child care personnel in attendance, and time taken for all individuals to complete the drill. Documentation of conducted fire and emergency preparedness drills must be available at the time of inspection. Documentation produced after the inspection will not be used to meet the licensing standard or corrective action requirements. ) Last fire drill was conducted on 9/20/2024, One fire drill using an alternate evacuation route was done on 9/20/2024, One fire drill in the presence and at the request of the licensing authority has not been done for new Licensing Year. Licensing Specialist observed that the facility did not have documentation proof that all child care personnel were trained and knowledgeable of the use of fire extinguishers within 30 days of date of hire. Technical Assistance was given to the Provider as per the SACCF Handbook that ( Fire Extinguisher Training: All Child Care Personnel shall be trained in the use and operation of a fire extinguisher, at each facility they are employed, within 30 days of date of hire. The facility must maintain documentation that all Child Care Personnel have completed training.)

Licensing Specialist observed that Four (4) CF-FSP Form 5131, Background Screening and Personnel File Requirements form was incomplete for Child Care Personnel. Technical Assistance was given to the Director as per SACCF Handbook that ( Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. )

12-01 · Facility Environment SACCF Handbook
Licensing Specialist observed several ceiling vents/fan areas of the facility were in need of cleaning. One in the boy's bathroom, located in the computer room , in the Lobby Area at the entrance of the Administrative Office and in the common areas. Technical Assistance was given to the Director as per the SACCF Handbook that ( All school-age program facilities must be clean, in good repair, and free from health and safety hazards and vermin. During the hours that the program is in operation, no portion of the building can be used for any activity that presents a risk to the health and safety of the children. Indoor and outdoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Any problems must be corrected before the play area is used by children. Documentation must be maintained for 12 months. It is the responsibility of the director/owner to ensure that all areas and equipment of the facility are free from fire hazards, such as lint and dust build-up in heating and air vents, filters, exhaust fans, ceiling fans and dryer vents. This includes grease build-up in ovens, stoves and food equipment. )

12-02 · Facility Environment SACCF Handbook
Licensing Specialist observed several areas of the facility were in need of repair. 1. There were several water spots on ceiling tiles in the Computer Room, Lobby/Entrance classroom, 2nd Classroom in front of the Food Preparation Area, and common areas/hallways throughout the facility. 2. Closet's Door Knob for A/C Unit, (located in the BIG Classroom in front of the Food Preparation Area) is broken, missing the inner lock making all contents accessible to the children in care. 3. The Air vent in the same room for the A/C unit mentioned above is loose and missing multiple screws making it a potential safety hazard for the children in care. 4. The Pantry Door in the Food Preparation Area is missing the lower door vent, leaving a large hole on the door with splinters and sharp edges to the children in care and also vulnerable to unwanted entrance to food stored when actual door is closed. 5. No electricity and exposed cables in the electrical outlet by the playground door's entrance/exit. License Specialist assured the exposed cables by adding protective cover with electrical covers and Director moving the book shelf in front of the outlet at the time of inspection to avoid accessibility by the children in care. 6. Boy's bathroom in the Computer Room: a. Contained peeling paint in the entire bathroom door. b. Multiple floor tiles were missing and broken at the entrance of the bathroom with sharp and jagged edges. c. The electrical outlet on top of the toilet which is across from the shower area, had no covers and did not meet GFCI (Ground Fault Circuit Interrupter) needed in areas of high moisture such as bathroom, kitchens etc. d. The BATH FACILITY/Shower in this bathroom is being used as storage. 7. The window in the Computer Room was missing all the interior sheetrock/Window sill, horizontal structure and surface at the bottom of the window, which serves to structurally support and hold the window in place. Refinishing needed for window structural support. Technical Assistance was given to the Director as per the SACCF Handbook that ( All school-age program facilities must be clean, in good repair, and free from health and safety hazards and vermin. During the hours that the program is in operation, no portion of the building can be used for any activity that presents a risk to the health and safety of the children. Licensing Specialist observed that there was no electricity/light nor a light fixture covering in the Bath/Shower/Bathroom located next to Director's office which S/A children, staff and guest use. Other Bath/shower available in the facility was being used as storage and the one available here is inaccessible since there is currently no light nor electricity. There was currently no shower/bath available for use at the time of inspection. Technical Assistance was given to the Director as per the SACCF Handbook that ( Every facility must have at least one portable or permanent bath facility available for bathing children. The bath facility must be in good operation, clean and must be sanitized or disinfected after each use. )

Licensing Specialist Observed a bag of fruit/apples in the refrigerator located in the Food Preparation Area. Label Stated: EZ Meals 2 Go Inc. 09/26 Step Up - 28 Fresh Apple Technical Assistance was given to the Director as per the SACCF Handbook that ( Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food- borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and not in bathroom facilities. Off-site food storage is permissible only if the site of storage is a licensed child care facility under the same ownership that includes a food preparation area that meets licensing standards. A. Food containers, such as cans, plastic containers, boxes, and bags, must be stored above the floor on clean surfaces protected from splash and other contamination. B. Food must be consumed or discarded on or before the expiration dates listed by the manufacturer. ) Violation was corrected at the time of inspection by the Director discarding of the bag with the remaining fruit/apples stored in the Food Preparation Area refrigerator

FSC observed various mishaps, when it comes to the fence area. The fence has plenty of gaps along the bottom of the fence area, also having the wires of the fence undone sticking out towards the inner area of the fence where the children's play in. FSC observed that attendance was taken on one roster. 3 children were observed in computer room with no attendance roster. Technical assistance was given to provider per SACCF handbook "Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom space (for example when the group goes outside for outdoor play, the child care personnel must take the attendance sheet/class roster with the group)."
