• Children in the outdoor play area were not adequately supervised in that [There was an incident at the school on 5/22. The class was at the playground. An unknown teacher walkeda child to the playground bathroom but then left the child unsupervised. The child ended up trapped in the bathroom unable to get out. The child was crying to get out to the dark room. She was in the bathroom unsupervised for 15 minutes
