The CCP listed on the Supplemental Sheet started working on 9/3/24 (over 90 days) & was supervising children with no other CCP in the classroom, and she has not completed the School Readiness OEL Pre-service training. Technical support was provided. 18 Training Requirements 18.1 Pre-service Timeframe All pre-service training requirements listed below must be completed by all program personnel, volunteers and substitutes, each as defined in this handbook, within 90 days of initial employment with any provider participating in the school readiness program. An accident report written on 4/10/24 did not have a parent/guardian signature for the child listed on the Supplemental sheet. The parent signed it during this inspection. Technical support was provided. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form
