Complaint alleges that mom had gone to pick up her daughter(CY DOB:6/2/23), aged one from daycare and realized after leaving the center the child's two fingers looked black and blue , so she went back inside and spoke to staff member Ms. Marie, who asked the teacher, Ms. Catia, what happened? The teacher stated the child smashed her hand and forgot to provide an incident report. However, the staff is willing to provide it the next day per parent . Parent also stated she called the owner Ms. Lisa to request footage of the camera, who stated she was not aware of the incident, but the cameras are for her office use only. Complaint inspection was conducted on 10/30/2024 and a statement from the acting director (MP) was received. Per the acting Director, the teacher went to the playground and left the child in the infant classroom because mom doesn't really like the child going outside because of possible mosquito bites. The teacher didn't see a mark on the child so an incident report was not created. She stated that she checked the child's fingers as well and didn't see any bruising. She told the mom on the date of incident at pick up time when she asked what happened to her child's fingers. Nothing was said to her beforehand. A video was shown to child care licensing and teacher was observed holding the child by one hand. It is not visible when the child's fingers were trapped by the door but the teacher is seen attempting to close the door once and then a second time. Then, she is observed walking out of camera view into another classroom. The accident/incident report was reviewed and it was dated 10/25/2024 and the incident occurred on 10/24/2024. The complaint is substantiated as the report was not created, signed and shared with the parent on the date of the incident
