Ratio were observed as follows: Infants 1:4, Toddlers 1:6, Older Toddlers 1:6, Twos 2:13, Threes 1:15, and Vpk 1:13. The Older Toddlers (6) of them, were observed sleeping, the teacher was not present at the time the monitor enters the classroom, both doors were closed. The teacher was located in the classroom beside the Toddlers room grabbing a sleeping mat, (both classrooms are separated by a door). The teacher came back to the classroom and technical assistance was provided

A complaint allegation was received alleging that child Mb was left unattended and unsupervised at Margate Christian academy with a pair of scissors in the classroom and managed to cut her hair , her pants , her shoes and blanket. An on-site inspection was conducted on 5/11/2023 and a statement from the teacher(TC) and office staff (KC)was received. Per teacher, TC, she was subbing the class since the assigned teacher was running late. It was circle time, and the child was misbehaving so she was asked to sit at a table. When she turned her attention to other children, she doesn't know where the child got the scissors from and didn't notice she started cutting up her clothes and hair until she started seeing her beads on the floor and children playing with them. The child was trying to hide the scissors but she saw them in the child's hand. She took he scissors away and informed the front office of what the child had done. An incident repot was not created on the date of incident and parent was notified at pick up time. This complaint is Substantiated as an incident report was not created for this incident on the date of occurrence. Per office staff, KC, there is no recording of the incident as it only stays for 24 hours and then it auto erases. Per the attendance and KC there were 13 children in attendance and classroom was within ratio. A walkthrough of the room was done with KC, she stated that the child took the scissors from the art area on the right side of the room when you enter the room. This complaint is Substantiated and inadequate supervision non-compliance is being issued as per the incident report and the teacher's statement, the child had access to scissors and was not seen when she started cutting up her clothes and hair

A complaint allegation was received alleging that child Mb was left unattended and unsupervised at Margate Christian academy with a pair of scissors in the classroom and managed to cut her hair , her pants , her shoes and blanket. The parent was not notified until they picked Malaya up. Malaya was dismissed with the same clothes she destroyed. Staff state there was a substitute and she was unaware of what Malaya was doing. No incident report was made on the day of. On May 4 2023 Malaya was sent home with a scratch on the side of her left eye and there was also a red bruising on her chest. The parent was not informed of any incidents that may have occurred that day. Staff stated they were unaware and no one acknowledged it or made an incident report on the day of. An on-site inspection was conducted on 5/11/2023 and a statement from the teacher and office staff was received. There was no incident for the May 4 2023 incident as the staff was unaware of the mark under her clothes. The child did not cry or mention anything to the teacher of how she could have gotten the mark. Per teacher, TC, she was subbing the class since the assigned teacher was running late. Two staff members were observed with a current CPR/First Aid certification on file, the facility did not meet this requirement as there must be at least five (5) staff members with the appropriate CPR/First Aid training. Technical Assistance was given to the provider Regarding New CPR/First Aid Requirements. 4.2.4(B) DCF Handbook, The facility shall maintain the following number of child care personnel with CPR certification and first aid training that are on site at the facility at all times children are in care. For child care personnel to count to meet requirement, they shall be child care personnel assigned to provide direct supervision of children in care: For 0 25 child care personnel needed to meet ratio requirements there shall be at least three (3) personnel with current CPR/First Aid For 26 50 child care personnel needed to meet ratio requirements there shall be at least four (4) personnel with current CPR/First Aid For 51 75 child care personnel needed to meet ratio requirements there shall be at least five (5) personnel with current CPR/First Aid For every additional 25 child care personnel there shall be at least one (1) additional staff person with current CPR/First Aid Documentation of pediatric cardiopulmonary resuscitation (CPR) certification and first aid training must be kept on file and available for licensing to review. Child care personnel satisfying the certification and training requirements shall be present at all times that children are in care at the facility.
