Complaint alleges that there are unsanitary toy and equipment sharing: Infants and toddlers share toys, playpens, and other items throughout the day without proper cleaning or sanitization between uses. FINDINGS: A complaint inspection was conducted on 10/21/2025 and a walkthrough of the infant room was conducted. The infants were observed playing with and mouthing toys. The teacher were observed putting toys away in the mat play area without separating the mouthed toys for sanitizing. This complaint is substantiated

Complaint alleges that the school's Playpen sheets are often not changed between children, even when visibly soiled. FINDINGS: A complaint inspection was conducted on 10/21/2025 and the following was observed; Four playpens and five infants were observed. Per director, there are 5 playpens, 4 in the classroom and one is kept in the hallway. After my walkthrough i did not see the 5th playpen. Then, the director was observed going to a vehicle and retrieving the 5th playpen. The director stated that she takes them home to clean over the weekend. There were 5 infants present on Monday and Tuesday and only 4 playpens present. This complaint is substantiated
