A complaint allegation was received alleging that child (J.G. DOB: 10/27/2023) suffered a fracture (as per hospital report), bruised and cut middle finger on her right hand due to being bit by a male classmate because they were fighting over a toy on 2/10/26 at approximately 11am.______________________________________________________ FINDINGS:Video of incident was reviewed and the following was observed; The teacher was washing her hands, sweeping the floors, and organizing the tables and chairs while the children sat on the carpet, playing with toys.________________________________________________________________________________________________ A statement from the teacher was received. Per the teacher, the incident occurred at around 11:00AM. The children were playing with blocks on the carpet and she was cleaning the table because the children had just ate. She heard J.G. crying so she went to tend to her and that is when she noticed that her hand was bitten. She did not see the incident, she just saw the mark when she responded to the childs cry._____________________________________________________________________________________________________This complaint is substantiated for inadequate supervision. TA was provided that teachers must be watching and directing the childrens activities within close proximity. A suggestion was provided to have an additional staff member clean the room while the teacher tends to the children

A complaint allegation was received on 12/6/2023 alleging that child NC was injured on two occasions, 11/13/2023 and 12/04/2023. First on 11/13/23 Child had a scratch on right cheek that was caused by another child and parent was not informed of the injury nor was an incident report filed. Second, on 12/04/23 child had various scratches on his forehead caused by another child and the parent was informed of the incident and signed an incident report. A complaint inspection was conducted on 12/11/2023 and a statement was received from the teacher (AC) and Director (DR). Based on the statement received form the teacher, she saw the incident happen on 12/4/2023 but she just couldn't stop the child on time. When she pulled the other child away, he had already scratched the other child. She was not present when the first incident that happened on 11/13/ 2023 and the other co-teacher was out of town. Accident/incident reports were checked for compliance. Accident/incident report from 11/13/2023 was complete and signed by parent. Accident/incident report from 12/4/2023 was written but not issued to parent on date of occurrence. This complaint is Substantiated for not providing an incident report to the parent on date of occurrence
