At the time of the inspection observed the diaper changing area with a tear. Advised provider of issue, Provider stated she has extra's located in her storage. Diaper Changing mat was replaced. Violation cleared, standard in compliance. Provided TA that Diaper changing table must always be in good repair and sanitized

Upon review of the provider's renewal documents which were received on 05/22/25 and 05/16/2025, we identified that the Staff credential expired 09/28/2022 According to the DCF Training Transcript. JJ contacted the provider K.B immediately to inform her of the issue on 06/03/2025. Provider stated that She submitted her AS Degree to the Registrar and that she was informed she did not need to worry about additional renewal. Per the conversation with K.B, C.D and J.J Provider was not aware of the proper procedures regarding the staff credentials renewal. We informed the provider what she needed to complete and inform her that she will be cited for the out of compliance and would have to attend the CCAC Meeting on June 11, 2025 @ 9am Provider agreed and stated she will be in attendance. She stated that she has started the process as of 06/03/2025 after a conversation with DCF. Informed provider we will continue to monitor the transcript. Article XIX(D)(4)(Viii) Large Family Child Care Home Operators. In addition to the training requirements identified in rule 65C-20.009(3)(a) and (c), F.A.C., large family childcare home operators must: i.
