At the time of inspection, the monitor observed children being transported to the after-school program by child care personnel and another driver. The licensing monitor immediately stated to the child care personnel, the facility does not have approval for transportation services. Transportation must cease until local licensing agency approval has been granted. Following progressive enforcement procedures, a Notice of Violation is being issued as this standard was previously cited out of compliance on 8/20/25. The providers license expires on 9/30/25. All areas of this report that fail to comply must be corrected by 8/22/25 or the renewal license may be denied
