At the time of inspection, the monitor observed open containers (mop buckets) of water were not emptied after use in multiple classrooms. The licensing monitor brought this concern to the attention of the teachers and the school director. The teachers immediately discarded the water bringing this standard back into compliance

The licensing monitor observed destination procedures were not followed in that upon arriving to the program the driver did not immediately mark off each child as they departed from the vehicle to verify all children were accounted for. While inspecting the additional vehicle the monitor had to prompt the school director to complete the second sweep as that vehicle was leaving. Technical Assistance: F. Upon arrival at the destination by vehicle, the driver of the vehicle must: 1. Mark each child off the log as the child departs the vehicle; 2. Conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and 3. Record, sign, and date the transportation log immediately, verifying that all children were accounted for, and that the visual sweep was conducted. 4. Ensure that a second child care personnel conducts a second visual sweep, signs and dates the transportation log verifying that all children were accounted for, and that the log is complete. 5. If the provider contracts with an outside entity to provide transportation, then the provider must assign a child care personnel to perform the duties of the driver outlined above in numbers 1-3. G. Upon arrival at the destination by vehicle, a second and different child care personnel must: 1. Conduct a physical inspection and visual sweep of the vehicle to ensure that no child is left in the vehicle; and 2. Sign, date and record the transportation log immediately, verifying that all children were accounted for, and that the log is complete

At the time inspection, the monitor observed office manager leave the front office area and removed a one-year old child from the 1's classroom into the infant classroom. Per the office manager, the child's parent just arrived and placed the child into the 1's classroom. The monitor verified the time the child was signed in on the classroom roster was 30mins prior to inspection. The monitor informed staff on site that the classrooms need to remain in ratio when the children arrive. Provider resolved standard by correcting room ratio and roster entry.
