The school director self reported that An incident occurred today at Greater Horizons Academy involving a child at the gym during playtime. The teacher was attending to another child who was acting up, and when she turned back around, she saw that the little girl was on the floor. The teacher went to check on her and noticed a scratch on her face and a bruise on her cheek. The teacher said she didnt know who did it because she was attending to another student. We called the parent and filed an incident report. The parent was upset and wanted to take further action, so I reported it on the DCF website. FINDINGS: A complaint inspection was conducted on 9/30/2025 and a statement from the teacher was received. Per the teacher, the incident occurred on Friday afternoon. The 2y/o were at playtime in the gym. She has an autistic child that is very active. When they go to the gym and they play with a ball they do not like to share. So he was acting up really bad, throwing a tantrum. When she turned around and looked as SM, the child was on the floor. When she picked her up she saw that one of the students scratched her up in her face. The teacher did not see what happened or who scratched the child. She just saw the marks on the child when she picked her up off the floor. The teacher was asked if there is a way for her to communicate to office staff when she needs help with challenging behaviors and she stated that she has to leave the room with the whole group. I provided TA to the director about using an efficient means of communication, like a walkie talkie to provide support to the staff. A non-compliance was issued for inadequate supervision as the teacher was not watching and directing the children's activities when the incident happened
