During the inspection, Licensing Specialist observed that there were 6 expired immunizations within the childrens file. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.1: The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A -1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C22.001(7)(p), F.A.C., from the custodial parent or legal guardian. 5-4: During the inspection, Licensing Specialist observed that there was one child signed up for the fieldtrip on 6/12/2024 (to Old McMickeys Farm) on the parent permission form and observed that the one child was not listed on the transportation log. The director advised that the one child did attend the fieldtrip and showed a photo of the child at the Old McMickeys Farm on 6/12/2024. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.2: A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable)

5-4: During the inspection, Licensing Specialist observed that all the transportation logs since 11/16/2023 did not have the signature for the second person conducting the visual sweep of the vehicle. Licensing Specialist also observed some transportation log entries were missing the drivers signature, some were missing the time on the vehicle and the time off the vehicle, and some had children check marked on the vehicle, but not when they got off the vehicle. 07-04 · Vehicle Requirements
During the inspection, the director advised that currently there are two transportation vehicles. The first vehicle is a 2013 Chevy (67476) that was last inspected on 11/12/2022 and is insured through Church Mutual Holding Insurance with an effective date of 2/1/2023. The second vehicle is a 2004 Chevy (13102) that was last inspected on 11/20/2022 and is insured through Church Mutual Holding Insurance with an effective date of 2/1/2023. 7-4: During the inspection, Licensing Specialist observed that the facility did not maintain the annual vehicle inspection forms for the 2013 Chevy (67476) and the 2004 Chevy (13202). Staff advised that they did not have a copy of the annual vehicle inspection on site. Staff called the mechanic and the mechanic advised that they did not have a copy of the vehicle inspections to send to the facility. Licensing Specialist advised that if they correct the non-compliances, then they can contact Licensing Specialist so that the reinspection can be completed sooner than the due date. 17-06 · Outdoor Play Area/Fencing
17-6: During the inspection, Licensing Specialist observed that the fence had gaps that exceeded 3.5 inches. The facility installed a mesh material around the facilitys exterior fence, however the mesh material was not in good repair in that there are tears in three sections of the fence exposing the 4 inch gap between the fence rails. The director advised that they are aware of the tears in the mesh material and are in the process of getting the issue resolved. The director advised that the company should have repaired the mesh material on 11/13/2023, however the company emailed the provider stating that the material has not arrived yet and that they will keep the provider updated on a new ETA. Licensing Specialist advised that if they correct the non-compliances, then they can contact Licensing Specialist so that the reinspection can be completed sooner than the due date. Additional time is being provided due to a delay in materials. During the inspection, Licensing Specialist provided technical assistance, The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3.5 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided
