During the previous inspection, the Licensing Specialist observed in the following room paint peeling Room 400 the wall by the door that leads 450, Room 350 the wall above the discovery shelf and the side, Room 300 the wall by the phone, under the fire alarm and above the soap dispenser the left wall, Room 150 the wall by room 200 and the School Age room the wall by the basketball hoops. The licensing specialist observed the contact paper that is on the shelves was peeling in the following rooms: Room 150, Room 250 and the School Age Room the shelf that is black and the ice hockey. The licensing specialist observed tiny holes in the bathroom walls in the following Rooms: Room 350, Room 300, and the school Age Room. The licensing specialist observed in the bathroom in the 200 room that the toilet paper holder was rusted and had a sharp edge. The licensing specialist observed the duct tape that is on the seats in the buses is peeling up. During the reinspection, Licensing Specialist observed that the provider painted where the paint was peeling, removed the shelves that had the peeling contact paper, removed the toilet paper holder that had rust and a sharp edge, and repaired the seats on the buses. During the reinspection, Licensing Specialist observed the tiny holes in the bathroom walls in the following Rooms: Room 350, and the school Age Room have not been repaired. The director was given until April 14, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 A.- General Health and Safety Requirements. A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

33-03 · Training Requirements
During the time of inspection, the Director advised the Licensing Specialist that there 33 employees. During the time of the inspection, the Licensing Specialist monitored 33 of the 33 employee records. REMINDER: In-service training for 2024/2025 fiscal year must be completed before July 1, 2025. Licensing specialist reviewed 10 hours of annual in-service, completed, for all child care personnel for the 2023/2024 fiscal year. During the inspection, Licensing Specialist observed 1 personnel that did not start her 40 DCF hours within the first 90 days in the industry. The director brought this standard back in compliance at the time of inspection by having the personnel start her classes. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 4.1 Beginning Training Childcare personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the childcare industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the childcare industry does not constitute begin training. The begin training timeframes begin at the time of employment in the childcare industry. Training completion may not exceed 15 months from the date of employment in the childcare industry in any licensed Florida childcare facility. This may be accomplished by classroom attendance in a department-approved training course, acquiring an educational exemption from a department-approved training course, beginning a department-approved online childcare training course, or by receiving results from a department-approved competency examination. 04-04 · Supervision
4.4- During the inspection, Licensing Specialist observed that cameras for the incident on 7/16/2024. Licensing Specialist observed class 300 is on the 300 & 350 playground with JR. JC is on the 300 & 350 playground. Class 400 is on the 400 playground with DS. JRs class goings inside at the 5 min 42 second mark with a second teacher. JR goes into her classroom at 6 min 27 second mark. JCs is sitting under the shade structure, alone on the 300 & 350 playground. At this time the fence gate is open and there is a teacher in the fence gate entry and is looking at the 400 playground and the 400 class is lining up to come inside. At the 6 min 44 second mark, the teacher walks away from the fence gate entry back on to the 400 playground. At the 7 min mark, some children are standing in the fence gateway with a second teacher. At the 7 min 44 second mark, a child goes to the 300 classroom door and then runs back to the fence gate. At the 8 minute 5 second mark, 2 children go the to 300 classroom door and then walk up to JC. The 2 children go back to the 300 classroom door and then walk back to their teacher, DS who is now standing in the fence gate entry. At the 9 min 3 second mark, DS walks over to the 300 classroom door and looks through the window. At the 9 min 25 second mark, DS has JC come with her into her classroom. 39.3-The director, owner and child care personnel stated that staff failed to document the incident were the child was left alone on the playground on 7/16/2024. 12-9: During the inspection, Licensing Specialist observed that there was evidence of insects in the facility in that there were 2 live roaches in Suite 200, 3 live roaches in Suite 250 and a number of dead roaches in the kitchen. During the inspection, the owner advised that they do have pest control that comes out monthly. The owner advised that they will have pest control come out again. 32-3: During the inspection, Child Care Regulation observed that there were two pieces of playground equipment that were not in good repair in that there was a plastic picnic table that had a crack in the seat were a child could potentially pinch their finger and the climber tunnel was had a crack along the screw the window opening were a child could potentially pinch their finger on the four year old playground. Additional time is being provide due to potential material shortage and potential financial hardship. During the inspection, Child Care Regulation provided technical assistance, All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. 45-7: During the inspection, Child Care Regulation observed one child care personnel that had an eligibility date in the Clearinghouse of 9/16/2022. The one child care personnel did not start the child care industry until she was hired at the facility on 5/30/2023. A re- submission was not completed when the one child care personnel was hired. 46-07 · Daily Attendance
46-7: During the inspection, Child Care Regulation observed that the classroom attendance roster was not accurate in that there were children signed in on the attendance, however they were not all in the classroom. In Suite 150, Child Care Regulation observed 9 infants in the classroom and only 5 children signed in on the roster. In Suite 250, Child Care Regulation observed 6 children in the blue group and 8 children in the pink group making it 14 children total however there were 15 children on the roster; the teacher advised that the one child was moved into a different classroom. In Suite 500, Child Care Regulation observed 5 children in the classroom and 11 children signed in on the roster; the teacher advised that the 6 children were at Soccer. In Suite 450, Child Care Regulation observed 6 children in the classroom and 10 children signed in on the roster; the teacher advised that 4 of the children were at Soccer. All of the children returned to their classroom, making the classroom rosters accurate. This is being marked corrected at the time of the inspection. During the inspection, Child Care Regulation provided technical assistance, Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. 39-3: The teacher stated that at around 4:50 pm, when the class was outside, the child came up to her and showed her that he had scratches on his arms. The child said that she scratched him when she grabbed him by his arms. The teacher advised that she did not realize that the child got picked up by mom and forgot to create an accident report for the child. The teacher advised that she got distracted signing children out as they were picked up. The owner advised that an incident report was not written on 4/11/2023 due to the injury not being noticed until the end of the day, so the teacher did not have time to write the report. The owner stated that the incident report was written the next day. This is being marked corrected at the time of the inspection. 47-01 · Access/Child Safety
47-1: Child Care Regulation reviewed the video of the incident on 4/11/2023. Child Care Regulation observed the child laying on the carpet, while the teacher is teaching the other children in the room are on the carpet. The child is seen moving his legs in the air. The teacher walks around a shelf and some of the children start to leave the carpet area. The teacher walks back to the carpet and picks the child up by the outside of his arms and leads him away from the carpet area. When the childs mom came to pick up him up, she put the child in his car seat. The child complained that he had a boo boo, and the mom saw that he had scratches on his arms. When Child Care Regulation spoke with the teacher, the teacher stated that on 4/11/2023, at around 11:40ish, she was teaching a large group on the carpet. The child was rolling on the carpet and the teacher told the child to stop however he kept doing it and she let him be due to the child looking for a reaction from the teacher. The teacher stated that children were trying to walk past the child and the child almost kicked them as they walked by. The teacher stated that she picked the child up and moved him away from the other children. The teacher stated that at around 4:50 pm, when the class was outside, the child came up to her and showed her that he had scratches on his arms. The child said that she scratched him when she grabbed him by his arms. The owner advised that the teacher did not pick the child up in an appropriate way (grabbing the child with two hands at the top of the childs arms) and that the teacher should have picked the child up under the armpits. The teacher completed a training on additional training on positive redirection (Course title: Keep Calm and Try Positive Guidance and Redirection by Pro Solutions). This is being marked corrected at the time of the inspection. During the inspection, Child Care Regulation provided technical assistance, Child care personnel must appropriately interact with children to foster a healthy, safe environment that will encourage the childs physical, intellectual, motor, and social development. Interactions with children that are aggressive, demeaning or intimidating in nature are strictly prohibited
