17-01 · Outdoor Play Area/Fencing
During the inspection, the Licensing Specialist observed on the middle playground, an area near the back left side of the fence (when facing the playground) across from the school building, there was an area, (which is not a fall/zone area, none of the structures in this playground are anchored into the ground) where artificial turf had come up and exposed the blue foam (the foam, was blue, white, gray, and black) underneath with pieces of foam coming off. During the inspection, pictures were taken. The director was given until April 10, 2025, to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook Section 3.5 A- Outdoor Play Area. A. The outdoor play area must be clean and free from litter, nails, glass and other hazards

17-07 · Outdoor Play Area/Fencing
During the inspection, the Licensing Specialist observed on the playground fence on the young playground (the playground to the far right when facing the playground) had a hole at the base of the fence larger than 3 1/2 inches deep. The whole was on the right side of the playground (when facing the playground), on the left side of the sprinkler system. The sprinkler was in the whole. The whole was 9 inches deep from the bottom of the fence to the bottom of the whole, and 10 inches wide coming away from the fence. On the older children's playground (the playground to the far left when facing the playground), there is a whole on the left fence on the left side of the playground at the bottom of the fence towards the back of the playground. The whole was 10 inches deep, and 10 inches wide when measuring along the fence. The director was given until April 10, 2025, to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook Section 3.5 F- Outdoor Play Area. F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build- up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided

05-04 · Transportation
5-4: During the inspection, Licensing Specialist observed some transportation log entries were missing the drivers signature, the secondary signature, the time on the vehicle and the time off the vehicle, some had children check marked on the vehicle, but not when they got off the vehicle, and some had children checked off the vehicle but not checked on the vehicle. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.2 A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable)

During the inspection, Licensing Specialist observed left over food (cantaloupe, sliced cheese, shredded cheese, mac and cheese, bagels, peas and pasta) that was sealed, however it was not labeled with the date that it was opened/cooked. Licensing Specialist observe opened packages of food that were not sealed (shredded cheese, nuggets, and Teddy Grahams). During the inspection, the food items were dated. This is being marked corrected at the time of the inspection. 23 10 hours of annual in-service training. 33-3: During the inspection, Child Care Regulation observed 1 child care personnel that did not start the DCF 40 hours of introductory training within 90 days of their child care industry start date. During the inspection, Child Care Regulation provided technical assistance, Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. 33-6: During the inspection, Child Care Regulation observed 1 child care personnel that did not complete the 5 hours of early literacy training within 12 months of their child care industry start date. During the inspection, Child Care Regulation provided technical assistance, Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. 04-01 · Supervision
4-1: During the inspection, Child Care Regulation observed the video from 4/20/2023. Child Care Regulation observed first teacher already outside on the playground, while the second teacher was still inside the classroom holding the door for the other children to go outside. The child is seen standing in front of the window. As the children are let outside, the first teacher moved the children to sit/stand in front of the window outside. All the children, minus the one child, walked onto the playground and the second teacher closed the door, leaving the child inside the classroom by herself. Once the door closed, the child ran over to the door and tried to open it, then the child went back to stand in front of the window. A teacher from the classroom next door opened the door between the two rooms. The child turned around but did not go toward the teacher. Another teacher came into the doorway and the child ran over to the teachers. The child went into the adjacent classroom. The child was in the classroom by herself for 45 seconds. Child Care Regulation spoke with both teachers and the teachers advised that they counted the children inside the classroom, however they did not count the children when they went outside. Both teachers advised that they did now that they left a child inside the classroom unsupervised until the office staff notified them. During the inspection, Child Care Regulation provided technical assistance, Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location
