Additionally, the counselor observed the facility uses one sheet/log for the Know Your Child Care Facility, Influenza and Distracted Driver brochures. The provider stated the acknowledgment for the Know Your Child Care Facility was electronic but was unable to locate or show the counselor at the time of the inspection. A suggestion was given to the provider to separate the roster/log to have the Influenza and Distracted Driver brochures on one sheet and to either incorporate the Know Your Child Care Facility brochure on the registration form or do a separate sheet for this brochure as it is given at time of enrollment
