At the time of inspection, the Licensing specialist observed the provider's annual fire inspection was completed on 5/9/2025. The facility's previous inspection was completed on 4/5/2024. The specialist advised the provider that the fire inspection must be completed annually. The inspection was completed but it was late. TA was provided to ensure that the facility completes an annual fire inspection. At the time of the inspection, Licensing counselor observed the providers last fire drill was conducted on _____5/9/2025________. The provider conducted a lockdown drill on _____12/19/2024________ and a weather drill conducted on _________. The fire extinguishers were last tagged on_May___ 2025. At the time of reinspection, the Licensing specialist observed the red metal fire truck with paint still chipping and needing to be repainted. The provider advised that they have fast drying spray paint and repainted it at time of inspection while the children were napping and will not go outside until later in the afternoon. The counselor advised that the provider must ensure that the equipment is dry prior to children accessing the playground. The provider stated she understood, the provider spray painted the equipment with quick drying paint at time of inspection. TA was provided to ensure that the equipment is in good repair and the potential hazard of paint chipping is eliminated

At the time of inspection, the counselor observed the facilitys playgrounds with the resilient surface of mulch not measuring to 6 inches in depth near stationary equipment. The counselor observed under the yellow swings attached to the wood equipment 2 inches of mulch, the stationary multicolored caterpillar tires on both playgrounds with 4 and 1 inch of mulch, the plastic climbing equipment with red slide with 5 inches of mulch and the green metal car with also 5 inches of mulch. 14-02 · Lighting, Temperature, and Ventilation
At the time of inspection, the counselor observed both the back and front VPK classrooms with tables used as workspace for children in care with lighting that did not reach 50 foot candle lighting. Also, a table in the three year old classroom was observed not reaching 50 foot candle lighting. The back VPK classroom measured lighting at one table with 36.4 lighting, the middle classroom measured at 41.5 lighting. Also, the last table measured at 40.9 lighting. In the three year old classroom the table near the back VPK classroom measured at 39.9 lighting. As well as in the front VPK classroom the table closest to the window measured at 33.7 lighting. The provider immediately rearranged tables and opened the curtains in the back VPK classroom and all tables measured to over 50 foot candle lighting at time of inspection. The provider also rearranged a table in the front VPK classroom to measure over 50 candle foot lighting. Also, in the three year old classroom the provider rearranged the table that did not measure to 50 to near an area that provided more lighting. TA was provided to ensure that tables and areas used to workspace to include reading, writing and painting have 50 foot candle lighting at all times

At the time of inspection, the counselor observed the children in the three year old classroom eating colored goldfish and water. The goldfish was stored in a plastic container with a white scoop to distribute. The counselor advised the provider that snack must include at least two different food groups and must also be nutritious. The counselor verbally offered suggestions on snack examples that include two different food groups. The provider stated that she understood. TA was provided to ensure that children in care are offered at least two different food groups for snack daily

25-07 · Meals and Snacks
At the time of inspection, the counselor observed the thermometer in the back refrigerator used to store children in cares lunch with a temperature reading at almost 50 degrees Fahrenheit. The counselor placed a new thermometer in the refrigerator door and the counselor waited let it reach temperature and rechecked the temperature and it read 63 degrees. The counselor checked the temperature later in the inspection with the infrared thermometer and it read 48 on the right side and 46 on the middle shelf. The counselor moved the thermometer to the top shelf towards the back of the refrigerator and remeasured the temperature towards to end of the inspection and the temperature read 36 degrees. The provider manually dropped the internal temperature to the coldest setting but placed it back on recommended once the thermometer accurately read the temperature. TA was provided to ensure that the thermometer accurately displays the appropriate temperature of the refrigerator at all times, which should remain at 41 degrees or lower at all times

At the time of inspection, the counselor observed the children in care in the two and three year old classroom eating snack. The children in care all had the same snack per classroom. The counselor asked the provider who provides the snack food item and the provider stated they are donations. The counselor asked the provider who donates the food item and the provider stated the parents. Per the director a student from each classroom donates the snack items for the week for their classroom. The counselor advised the provider that the facility needs a snack menu posted at the beginning of each week for the snack items that we provided per classroom to include am/pm for all snacks provided at the facility to ensure all parents know what the child was offered. Parents provide childrens lunches daily from home. The director also advised that the snack food item can be different per day and that parents are only able to donate food items that we prepackaged in the original packaging and are sealed. TA was provided to ensure that the facility has a snack menu posted each week reflecting the actual food items being offered to all children in care

At the time of inspection, the counselor observed in the two and three year old classroom children that were napping but were not 18 inches apart. Counselor advised the staff that the children needed to be 18 inches a part. Counselor suggested that the staff move the table furniture over to allow more floor space. Staff moved the children further apart once the table was moved and more than 18 inches were from child to child at time of inspection. TA was provided to ensure that the children in care are spaced 18 inches apart while napping at all times
