At the time of the inspection the interviews and video footage revealed the teacher kicked the cot, away from her with the child was standing up on it during naptime while he was already crying. Due to the teacher kicking the cot, the child fell forward and scraped his head on the cot, causing a red scrape on his forehead and leaving mark on it. The facility was advised to ensure staff is not forcefully moving cots while children are on them to avoid inappropriate interaction. The teacher was suspended for the duration of the investigation and the child is no longer in attendance

At the time of the inspection the specialist observed no locking mechanism in the one year class, and a VPK class room. In the unlocked cabinets, the specialist observed chemicals such as Fabuloso and water mixed in a bottle, container of Chlorox wipes, water and bleach in a bottle, and glade air freshener. One of the cabinets was able to be attached to lock, but one remains unlocked. The provider needs to install a lock to prevent the children from having access to the hazards in the cabinet.The provider was given a due date, and has until that time to replace the locks to return to compliance. At the time of inspection, the counselor observed the facility with multiple ceiling tiles with stains, possibly caused by water damage. The provider confirmed that they do not currently have a leak and will replace the ceiling tiles. The counselor also observed the toilet in the first three year old classroom near the kitchen with feces smeared in the toilet seat and bowl. During this observation the staff in the classroom was on her phone and the children were sleeping during nap time. The air circulator vents in the restroom of middle three year old classroom was observed as well with dust build up as well as the back of the restroom door was stained with hand prints from children and paint. TA was provided to ensure that the facility is cleaned by the due date

At the time of inspection, the counselor observed in the middle two year old classroom to the left from the entrance a mop and bucket filled with water and a cleaning solution stored near the door. The staff was observed standing at the other end of the classroom and the children were sleeping during nap time. The counselor advised that there should be no standing water in the classroom with access to the children. The provider advised the staff to mop the floor and discard of the water after use. The first three year old classroom near the kitchen the counselor also observed a mop and bucket of water and cleanser stored in the childrens restroom. The counselor advised that after use the water should be discarded to eliminate a potential hazard. The provider immediately discarded of the mop water at time of inspection. TA was provided to ensure that potential hazards are eliminated and no standing water is ever accessible to children in care

At the time of inspection, the counselor determined that the child care personnel listed on the supplemental did not start a course within 90 days of starting in the industry. The staff started in the industry on 4/25/2024 and has not started a required DCF training course. The provider stated that the staff is going to challenge the training course and test out to obtain 40 hours. The counselor advised the provider that the staff needed to test out and pass at least one of the required courses to be in compliance within her 90 days of working in the industry. At the time of inspection, the counselor observed the three year old classroom with a hole in the wall to the right side of the classroom. The provider also observed paint missing on the first door to the right leading to the four year old classroom on the interior of the door as well as the last one year old classroom the wall near the playground exit on the left has paint missing. The provider advised that they will paint the areas missing paint and that need to be repainted as well as repairing the wall. Also, the counselor observed the ceiling tile in the middle one year old classroom broken with a crack that needs to be repaired. The whole milk gallons of milk were expired as of 4/29/2024 and the Honey maid graham crackers were expired as of 4/26/2024. TA was provided to ensure that the provider follows the manufacturers recommendation for expiration dates

At the time of inspection, the counselor observed the children in care eating different food items then what was posted on the facilitys menu for today. The provider advised that they had to offer substitutions. The counselor advised that the menu must reflect the changes made with substitutions before the food is served to the children in care. The provider stated that she understood and instructed staff to note the substitutions made on the menu posted in the kitchen and the lobby area where parents can view. The menu read children were having Sliced turkey, green beans, mashed potatoes, wg bread and milk but the children were actually served sliced ham, peas, rice and beans and milk. TA was provided to ensure that changes to the menu are noted and posted prior to the food being served to children in care

At the time of inspection, the counselor observed multiple childrens bottles and sippy cups that were not labeled and or labeled with the childs first name only. At the time of inspection, the counselor observed the child care personnel in the one year old classroom to the far right near the exit door leading to the playground change a toddlers diaper, wipe the mat with a baby wipe and not wash her hands, disinfect the mat, nor wash the toddlers hands. The staff proceeded to place gloves on to serve the children lunch. The counselor stopped the staff and advised provider that she needed to wash her hands, the childs hands and use a disinfectant to wipe down the surface. The staff did, then continued to serve the children lunch. TA was provided to ensure that staff follows the diaper procedures posted at all times

Counselor advised that chemicals and disinfectants should be kept high up and or locked away out of reach of children in care. Counselor also observed disinfectant wipes on a low shelf and in a cubby near the floor that was in reach of children in the back VPK classroom. Staff immediately moved toxic chemicals high up and out of reach of children

Counselor observed toddlers eating, meatballs, sliced mango, breakfast corndog and whole chicken nuggets that were not cut up to the appropriate size. Licensing advised that if parents do not cut up the food items for toddlers than staff will need to ensure the meals are age appropriate.
