At the time of inspection, the Licensing specialist observed a ceiling tile in the front preschool classroom near the office that was stained with a wet spot near the childrens restroom. Also, the specialist observed a few air conditioning vents with dust build up and some ceiling tiles with dust clusters on the tiles and needing to be cleaned. The vents were observed in the middle VPK classroom as well as the front preschool classroom. In the back three year old classroom the specialist observed a rust like stain on the metal slates holding the ceiling tiles. TA was provided to ensure that the facility is clean by the due date

At the time of inspection, the Licensing specialist observed the hand washing sink with under sink cabinet storage with the door broken and unable to latch shut, this caused the lock to be inoperable. The specialist observed several spray bottles storing different chemicals used for disinfecting in the cabinets in both VPK classrooms. The specialist advised the provider that the cabinet door was unable to lock shut and the provider advised staff to remove the chemicals and place them in another area that was not accessible to children in care. The provider also advised that they are working to get the sinks and cabinet units replaced because they are unable to fix them. The provider explained that the chemicals/ cleaning supplies would remain in another area until the cabinets are replaced. TA was provided to ensure that toxic and hazardous materials are not accessible to children in care

The counselor also observed the freezer in the kitchen that had debris present on the bottom of the freezer inside and also on the door. At the time of inspection, the counselor observed both one year old classrooms eating lunch and toddlers were provided a god portion of a half sized banana. The counselor observed children in both classrooms with the whole sized banana in hand shoving the entire piece in their mouths. One of the children in the young one year old classroom was observed gagging. The counselor asked the staff to cut up the childrens bananas at time of inspection to eliminate a choking hazard. The staff immediately cut up the toddlers food at time of inspection. The counselor and provider reviewed the section of the handbook related to food size for toddlers and infants together and counselor answered all staffs questions. TA was provided to ensure that food items for infant and toddlers are appropriate and do not pose a choking hazard

At the time of the investigation, information was obtained that revealed a teacher was supervising children did not know what happened to the child until she was notified by another child. Once notified she did not adequately supervise the child to respond to his needs. He cried and held his hand. The teacher noted that he favored his arm and even when he played his arm was bent. He would place his other hand on top. The teacher never sought help for the child or notified the parent so she could obtain help. The parent sought help after picking up the child and him crying in pain and he was unwilling to use arm. Parents questioned staff after dislocation of arm diagnoses and that is when the teacher recalled another student told her that someone jumped on the child. The noncompliance was resolved because the child is no longer in care

Information obtained during the investigation revealed the teacher did not document injury of child that was injured by a peer jumping on them during naptime. The teacher admits to noticing the child continuously saying hand to indicate that it hurt and him favoring his arm to keep it from hurting from after naptime until he was picked up. Not until she was questioned by the mother and director about the child's arm did she recall the incident. An incident report was still not documented of the injury at the start of the commencement of the investigation. The noncompliance was resolved because the child is no longer enrolled. The provider was advised to ensure all injuries/incidents are documented on incident report to maintain compliance of this standard

At the time of inspection, the counselor observed paint peeling on the wall in the back two year old classroom to the right under the window seal. Also, the counselor observed paint peelings in multiple areas in the first preschool classroom off from the office near the windows in the middle of the classroom and near the emergency door exit paint is missing. Provider advised that they used command strips and when they were removed they removed the paint from the walls. The counselor also observed in the same classroom the inside window panel with a hole in the frame where dry wall was cracked and loose in the right side corner. TA was provided to ensure that the entire facility is in good repair by the due date

At the time of inspection, the provider observed a child in the three and four year old classroom drinking water from the faucet of a hand wash only sink in the classroom after children came back into the classroom from the playground. The counselor advised provider that the child can not drink water from the faucet or any child in care. That children should be provided clean and sanitary drinking water while in care at the facility. Provider stated that they always used water from the faucet to offer children daily. The provider immediately grabbed extra bottles of water she had stored and also later sent staff to the store to grab gallons of water for the rest of the day as well as rest of the week. TA was provided to ensure that the facility offers children clean and sanitary drinking water at all times

At the time of inspection, the counselor determined that the child care personnel listed on the supplemental did not have an eligible status on background screening from the Clearinghouse documented on file and was left alone with children in care. The provider advised that the staff lived out of state and was awaiting the criminal history check from the Clearinghouse. Counselor remined the provider that the provisional letter clearly states that as a provisional hired child care personnel must be supervised by another fully screened individual and must never be left alone with children without an eligible screening status in the Clearinghouse. Today the counselor observed the child care personnel with the four and five classroom at time of ratio as the only staff in the classroom. The counselor advised the provider that staff does not have a eligible screening and can not be left alone. The provider had staff complete training online and also moved her to a classroom with staff who has an eligible status for background screening at the time of inspection. TA was provided to ensure that staff has an eligible screening in the Clearinghouse by the due date

At the time of reinspection, the counselor observed on the older childrens playground the red barrel tunnel with 4 inches of resilient surface of mulch and the green stationary merry go round was observed with also 4 inches of mulch. The provider immediately raked mulch from the tree area with the outdoor bench to meet the 6-inch requirement at time of inspection. Counselor observed that the provider laid mulch on the gated area in between the two playgrounds that has no equipment with mulch laid recently, counselor advised provider that mulch is not needed in that area unless they are planning to add equipment. TA was provided to ensure that the resilient surface of mulch is maintained at the requirement of 6 inches in depth and 6 feet around near all stationary equipment at all times

At the time of inspection, the provider observed the children in multiple classrooms not sleeping 18 inches apart. The counselor observed the nap requirement not met in the three year old classroom, the two year old classroom on the back left corner of the facility as well as the four and five year old wrap classroom. The counselor advised the provider that children have to maintain at least 18 inches apart at all times while napping. Counselor advised that the provider map out a classroom nap chart to ensure that children napping are spaced out to ensure a gap of at least 18 inches between children. TA was provided to ensure that the children in care do not sleep up against furniture as a safety precaution

At the time of inspection, the provider observed the children in multiple classrooms sleeping up against furniture and underneath furniture in the classrooms. The counselor observed the nap requirements not met in the three year old classroom, the two year old classroom on the back left corner of the facility as well as the four and five year old wrap classroom. The counselor advised the provider of the requirement and the provider stated that she understood. The provider commented that their corporate office does not like the furniture to be moved from the planned outline they provided. Provider stated that the staff will need to move furniture daily during nap time and move furniture back after nap time. Counselor advised that the provider map out a classroom nap chart to ensure that children napping are clear of napping in areas where furniture is staged. TA was provided to ensure that the children in care do not sleep up against furniture as a safety precaution

At the time of inspection, the counselor observed the resilient surface on the older childrens playground that did not meet the 6 inches in depth and 6 ft in perimeter requirement. The counselor observed all outdoor stationary equipment with less than 6 inches of mulch as the resilient surface. Provider produced an email from corporate representative detailing that mulch was ordered on 9/29/2023 to total $7,000 but delivery is expected within 4 to 8 weeks. TA was provided to ensure that the provider maintains the resilient surface is maintained for all stationary equipment at all times

At the time of reinspection, the counselor determined that a child in the one and two year old classroom had symptoms of vomiting and vomited twice while in the facility earlier in the morning and was observed at nap time in the classroom and was not removed after the first occurrence of symptoms shown. Counselor overheard the staff in the classroom talking of the ill children. Counselor asked about the sick children and if they were currently in the classroom. The staff replied that they had 3 ill children today with symptoms of vomiting and diarrhea and that two of the children had been picked up and the other child was the child observed sleeping the classroom at nap time. Staff advised that the office has been contacting the parents as well as the emergency contact for hours, but they received no response. Counselor asked office staff about the progress in reaching a parent and the staff stated at that time they talked to the parent 40 minutes ago and mom said she was on her way. Counselor advised staff to call mom after an hour had pasted and they did but mom did not answer. Counselor advised the provider that the child needs to be removed from the classroom and placed in the isolation area away from the other children. Staff immediately removed the child during nap time. After nap time the child was supervised by an additional staff into an empty classroom to have snack. At the beginning of snack time the parent picked the child up. The staff spoke to the parent and reminded them of the policy of pick up within an hour as well as the importance of keeping the other children in care safe. TA was provided to ensure that the facility follow their policy and utilize the isolation area as needed to keep all children in care safe
