At the time of the inspection, the facility did not have a daily indoor/outdoor checklist. A sample daily checklist was reviewed with the provider and a copy emailed on today's date for use going forward. The provider was reminded that indoor and outdoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log and any problems must be corrected before the play area is used by children

The baseboards under the highchairs where the infants eat needed to be cleaned from food debris. At the time of the inspection the counselor observed the infant teacher zipping an infant into the calming sleeping gown. She then grabbed the child by their leg and arm while they were on their back, flipped them to their stomach and dropped them to the mattress from the top of the crib. The teacher was asked if she needed a break to relieve her from immediate stress by the counselor to which she replied, she did not need this right now and continued to throw stuffed toys in frustration. The noncompliance was resolved once the counselor immediately told administration on duty to relieve the teacher for break
