At the time of the complaint, information obtained through interviews revealed a child in the infant classroom was fed formula for a bottle that was not hers. The noncompliance was resolved once the child was given the appropriate bottle; no longer enrolled. Provider advised to ensure all children enrolled are given their correct bottles/sippy cups to maintain compliance of this standard

At the time of the inspection the specialist observed one vent in the infants room that is in need of cleaning. The provider has until the due date to have the vent cleaned and return to compliance

At the time of the inspection, there were four inches of mulch around the green stationed car in the second playground. The provider placed more mulch around the green stationed equipment. The mulch met the requirement of six inches of protective surface around stationed equipment therefore it was completed at the time of the inspection. TA: The provider will need to rake the mulch in and around the large climbing equipment

The fire inspection for last year was conducted on 3/30/23 and expired 3/30/24 and then was not completed again until 4/5/24. Fire drills were conducted on a monthly basis and a fire drill was conducted with the counselor at the time of the inspection

At the time of the inspection, the CCR counselor observed the blue and red stationary car located under the tree on the far-right side had 2 sections of wood that were rotten and/or had a piece missing. Also, the blue and red shade structure had multiple pieces of wood that were rotten and 1 had an exposed screw

At the time of inspection, the counselor determined that provider's new staff, child care personnel listed on the supplemental did not have documentation of fire extinguishers training. Provider produced training certificates for new staff at the end of inspection that were completed at the time of the orientation onboarding process for new hires
