12-03 · Facility Environment
On 5/27/2025 the Licensing Specialist nearly tripped over an open can of soda placed on the floor near the entrance to the classroom area. The director provider cautioned the Licensing Specialist to be careful not to kick over her open soda. During the visit, the Licensing Specialist also noted additional concerns, including a used container of lemonade left on a table and the presence of other open beverages. It was observed that the director provider was consuming the drink while actively caring for children in the classroom area. The Licensing Specialist advised the director provider that consuming beverage(s) and leaving open drinks in various areas of the childcare facility is not in compliance with current health and safety standards. In response, the director provider explained that she keeps sodas and other beverages nearby due to her medical condition. The 2021 CCF Florida Handbook states the following and additional TA was provided. 3.9.3 Food Safety: Handling of food in a safe and careful manner prevents the spread of bacteria, viruses and fungi. Outbreaks of foodborne illnesses have occurred in many settings, including childcare facilities. Corrective Action: Additionally: The Licensing Specialist advised that open drink containers are violating health and safety standards outlined in the Child Care Facility Handbook under food safety and general cleanliness rules. Section 3.9.3 Food Safety, which requires childcare staff to follow proper food handling practices to prevent contamination. Although the specific act of drinking a soda from the floor is not directly listed, it conflicts with the expectations for sanitary food practices and modeling appropriate hygiene in a childcare environment. The Director discarded the can drink and relocated the container of Lemonade not accessible to the children therefore this standard was resolved at time of inspection 5/27/2025

13-03 · Technical assistance was provided regarding the proper storage of hazardous materials to ensure they are kept out of children's reach, in accordance with safety regulations-2021 CCF handbook 3.2, A and B that states the following. 3.2 Toxic/Hazardous Material/Firearms/Weapons A. All areas and surfaces accessible to children must be free from toxic substances, bio contaminants, and hazardous materials/equipment/tools, including power tools, plastic bags, matches, candles, lighters, etc. B. 43-02 · Technical assistance was provided to ensure compliance with documentation standards for enrolled children. TA was provided as follows Section 7.3 (Section 2.1 Health and Safety, Page 3): 7.37.3 Enrollment Information The facility operator shall obtain enrollment information from the childs custodial parent or legal guardian prior to accepting a child in care. This information shall be documented on CF-FSP Form 5219, Child Care Application for Enrollment, which is incorporated by reference in 65C-22.001(7)(f), F.A.C., or an equivalent form that contains all the information required by the Department on CF-FSP Form 5219. The Licensing Specialist observed classroom #2 to be out of ratio during the time of inspection, having a one-year-old child in the room of a total of eight children. TA was provided; The Florida Handbook states the following: 2.3.1 Mixed Age Groups B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one childcare personnel must be responsible for no more than six children of any age group, at all times. Ratio Room #1 Infant Room 1:4 Room #2 Mixed Group 1:8 (#1 one-year-old, #1 2-year-old, #2 three-year-old, #4 school agers) Corrective Action: The Director/owner returned to the childcare facility ensuring the correct ratio was maintained in room #2. 20-06 · Crib Requirements
During the time of renewal inspection on 5/30/2024, the Licensing Specialist observed that one of the cribs had a blanket and stuffed animal inside the crib area. The Licensing Specialist provided TA about Safe Sleep Practices and the employee/staff removed all items out of the crib. The Florida Handbook states the following about Safe Sleep Practices. 3.6.4 Safe Sleep A. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. 23-05 · Fire Drills
The Licensing Specialist went on renewal inspection on 5/30/2024 and provided TA that the Fire Extinguisher was due to expire in several days however upon return visit on 6/3/2024 the Licensing Specialist observed that the Fire Extinguisher had not been serviced and tags had expired. The Licensing Specialist provided the following TA from the Handbook. 3.8.2 Fire Safety B. Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times. NON-COMPLIANCE STATEMENT: The Licensing Specialist will give the provider until 6/13/2024 to get her Fire Extinguisher serviced and tagged and the provider will show proof that it has been completed via email or face to face re-inspection

The Licensing Specialist observed the Bleach and Pine Sol products were sitting on the counter in the area where food is stored and prepared for the children. The Licensing Specialist observed food sitting on top of the stove in the same area as the poisonous/toxic cleaning products. The Licensing Specialist provided the following TA about food and storage as follows. 3.9.2 Food Storage C. Poisonous/toxic chemicals or cleaning products must be stored separately from food. Products must not be stored on shelves above food preparation areas and/or food products intended for human consumption, unless placed in bins that are impermeable. The Licensing Specialist observed that the provider had moved all items that were poisonous/toxic cleaning products out of the food preparation area. Therefore, this standard is now in compliance

Upon time of inspection on 5/30/2024 the Licensing Specialist observed that there were no Diapering Procedures posted and the Licensing Specialist provided TA to the staff and provider/owner. The Licensing Specialist returned to the CCF on 6/3/2024 and no diapering procedures were posted in the infant room. The Florida Handbook states the following. 3.10.2 Diapering Requirements L. Diaper changing procedure must be posted in the changing area and followed to protect the health and safety of children and childcare personnel. Upon renewal inspection on 5/30/2024 the Licensing Specialist was unable to review the daily attendance records and gave the following TA from the Florida Handbook as follows. 7 Record Keeping Each of the records described in this section must be maintained at the program location and must be available during the hours of operation for review by the licensing authority: 4. Daily attendance of children records must be maintained for a minimum of 12 months. On 6/3/2024 upon re-visit the Licensing Specialist observed there was still no attendance in the classrooms of the childcare facility and the above TA was provided again to the directory/owner. The owner stated that she had run out of ink therefore could not print any attendance sheets; however, the Licensing Specialist did advise that each teacher/staff/employee who has a classroom can write out the attendance for the classroom. Corrective Action: The Licensing Specialist will have the Attendance Sheet for the month of May 2024 until current. This can be emailed to the Licensing Specialist or can be completed face to face re-inspection by 6/11/2024

Upon entering the Childcare Facility (CCF), the Family Service Counselor (FSC) observed the following ratio non-compliant. The infant ratio should be 1:4; however, one staff member cared for a total of (5) children. They were a mixed group that had one 4-month-old, one eight-month-old, two 3-year-olds, and one 4-year-old. Therefore, the following ratio was: Ratio 1:5 The Florida Handbook states the following. 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one childcare personnel must be responsible for no more than four children of any age group at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one childcare personnel must be responsible for no more than six children of any age group at all times. Corrective Action: The licensing agent provided TA and reviewed the ratio standard according to the Florida Statutes. The staff stated that she was aware that she was out of ratio at the time of the inspection, and minutes afterward, a parent picked up her 3-year-old daughter during the time of inspection; therefore, compliance to ratio was satisfied, making the ratio: 1:4 (mixed group one 4-month-old, one eight-month-old, one 3-year-olds, and one 4-year-old.)

46-01 · Daily Attendance
The FSC that this CCF did not have a daily attendance for 1/25/2024; therefore, the licensing authority was unable to review. The Florida Statutes state as follows: 7.5 Daily Attendance The childcare personnel must take and record the children's daily attendance accurately, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the childcare personnel and the group of children throughout the day should they leave the classroom. The FSC also provided further TA that stated that the documented attendance must be kept for one year for counselor review. The Florida Statute is as follows. 7.4 Keeping Record 4. Daily attendance of children records must be maintained for a minimum of 12 months. Corrective Action Completed The staff completed the daily attendance while in the presence of FSC, and FSC also observed a child leave the CCF during the inspection. The child was appropriately signed out, and the parent also provided a signature for the out of the child. The daily attendance had been corrected while onsite during the inspection on 11/25/2025

47-06 · Access/Child Safety
Upon arrival of the inspection on 1/25/2024, the director was out of the CCF; upon calling the owner/director, she reported that she had left and locked the staff and children files in the office; therefore, the FSC did not have access to the files needed to review for inspection. There was one staff on the premises; however, she did not have access to the files needed. The Florida Statutes report the following. 7.4 Records must be maintained and kept current on all childcare personnel, as defined by Section 402.302(3), F.S. These records shall be on-site and available for review by the licensing authority. Corrective Action Completed The owner/director later provided staff/employee and children's files for the FSC to review. The FSC reviewed all staff and children files at the CCF; therefore, this CCF is back in compliance
