Licensing Specialist observed one dog present that had immunization records present during inspection, showing that the dogs vaccinations expired 10/19/2025. Licensing Specialist gave technical assistance from the Family Day Care Home and Large Family Child Care Home Handbook during inspection: 7.1 Animal Vaccinations Animals, pets or fowl must be properly vaccinated, if vaccinations are available for the type of animal, pet or fowl; and be free from disease. Animals that are poisonous, aggressive, venomous, or pose a potential threat of harm to children in care are prohibited. Custodial parents or legal guardian must be informed in writing of all animals on the premises of the home. Such information may be provided by way of a parent flyer, a notification statement, or a statement included in the childs enrollment form. Documentation of current immunizations must be available for review upon request by the licensing authority. Corrective Action: Provider is obtain current immunization records for the dog & submit documentation to licensing specialist via e-mail. Provider is to submit via e-mail to Licensing Specialist proof that custodial parents or legal guardians were made aware of the dog being on the premises of the home

Licensing Specialist observed one fire extinguisher that did not have a current certificate during inspection. The certificate observed during inspection expired on 3/2026. Licensing Specialist gave technical assistance from the Family Day Care Home and Large Family Child Care Home Handbook: 7.11 Fire Safety B. The home shall have an operable smoke detector(s) and fire extinguisher with a current certificate. Corrective Action: Provider is to obtain a current certificate for the fire extinguisher & submit a photograph of the current certificate to the Licensing Specialist via e-mail

LS could not observe the hours that the substitute worked due to the hours not being documented. LS gave technical assistance from the FDCH & LFCCH Handbook: 3.2 Substitute The operator must have a written plan to provide at least one substitute, 18 years of age or older, to be available on a temporary/emergency basis. C. Any changes to the plan must be reported to the licensing office within 5 working days.
