29-02 · Health and Sanitation
Licensing Specialists observed one staff member in the infant room that changed an infant, did not wash the infants hands, placed the infant in their crib. Then the staff member washed off the diaper changing pad, did not wash their hands, and was observed to spray the child sized table to be cleaned. Licensing Specialist stated to the one staff member that there was no observation of the infants hands or the staff members hands being washed after diaper changing took place. The one staff member stated to the Licensing Specialist that she did wash her hands. Licensing Specialist observed 4 sippy cups present in the sink that were dry and observed the basin of the sink to be dry too. The staff member stated that she washed her hands and dried the sink because she does not like the sink to be wet. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 3.10.1 Handwashing Child care personnel and children must wash their hands thoroughly with soap and running water, dry, and follow personal hygiene procedures for themselves and while assisting others. Examples of activities when handwashing is required include but are not limited to: before and after eating, immediately following outdoor play, after toileting, following the use of any cleaners or toxic chemicals, before and after administering medication, and during food preparation and snack distribution. Corrective Action: Licensing Specialists spoke with director during inspection. The director had the one staff member wash her hands at that moment. Licensing Specialists observed the staff member was their hands during inspection. Therefore, standard was completed at time of inspection

Counselor was informed via e-mail by provider on 7/2/2024 that the director was no longer employed with the facility. The facility does not currently have a credentialed director. Counselor placed the facility on a provisional license beginning July 2, 2024 until January 2, 2025. Counselor gave technical assistance during inspection from the Child Care Facility Handbook: 4.7 Director Credential Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license. An applicant for the Director Credential must meet the requirements referenced in CF-FSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which is incorporated by reference in 65C-22.001(7)(k), F.A.C. CF-FSP Form 5290 may be obtained from the Departments website at www.myflfamilies.com/childcare. All applications and documentation will be verified, and, if complete, the credential will be issued by the Department or its designated representative on CF-FSP Form 5252, Florida Child Care and Education Program Director Credential, which is incorporated by reference in 65C-22.001(7)(g), F.A.C. Corrective Action: Provider is to obtain a director with an active director credential for the facility by 1/2/2025. Provider is to inform counselor and submit personnel records to counselor via e-mail once the facility obtains a director with an active director credential via e-mail

45-02 · Background Screening Documents
Counselor observed one staff member with employment date at the facility of 6/25/2024 that had a background screening clearance letter eligibility date of 2/22/2024 on file. Staff member had a 90 day break in-service from the background screening clearance letter eligibility date of 2/22/2024 and employment date at facility and in the child care industry of 6/25/2024. Counselor gave technical assistance from the Child Care Facility Handbook: 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. Corrective Action: Provider initiated a resubmission during inspection for the staff member. Therefore, standard was completed at time of inspection
