13-03 · Toxic Substances, Hazardous Materials and Weapons
Counselor received a complaint about an infant child that had redness in the eye or injury to the eye because of an activity done in the classroom. The infant room teachers and facility staff stated that the infant room had a sensory activity planned for the class. They had filled up a bucket with soap and water for play. During the activity, an infant somehow got soap in their eyes during the activity. The soap provided for play was "Assure" brand antibacterial skin cleanser with the active ingredient of 0.13% of Benzalkonium Chloride. The soap has in bold letters a warning label saying, "Keep Out of Reach of Children" and "Avoid Contact with Eyes". The staff failed to provide a barrier or protection for the children's eyes and did not follow the warning label on the soap used causing an infant child to receive an eye injury, scratch on the cornea, and some redness and swelling on their eyes. This has been resolved. The child was taken to the Emergency Room, and the provider threw away the soap that was being used. The provider has also put in place a new Handbook and Guidelines for the staff and how to avoid this from re-occurring. 3.2 Toxic/Hazardous Material/Firearms/Weapons B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times

39-01 · Accident/ Incident Notification and Documentation
Counselor received a complaint concerning an infant child that injured their eye following a class experiment and or a sensory activity involving soap and water. The label on such soap product has a warning to "Keep Out of Reach of Children" and "Avoid Contact with Eyes'. No barrier or protection was used to protect the children's eyes from the contents. Staff did notice the redness in the child's eyes and tried to rinse the eye with water solution. The staff proceeded by laying the child in a crib. When the parent arrived about 15 minutes later, the parent was upset to see the child's eyes red. The parent took the child and laid them on the changing station to rinse the eye some more. The parent was very concerned and took the child to the Emergency Room. Staff failed to provide an Accident/ Incident report for the parent. Parents did not sign anything stating what had happened. The condition of the child's eyes worsened after they left the facility. This matter was resolved when the facility drafted a report while the parent was in the Emergency Room with the child. The facility thought it was necessary after seeing the images of the child's eyes in a text message sent from the parents. 6.3 Emergency Telephone Numbers B. For life threatening injuries, the provider shall call 911 and then notify the parent. C. Custodial parents or legal guardians must be notified immediately if the event of any serious illness, accident, injury or emergency to their child, and their specific instructions regarding action to be taken under such circumstances must be obtained and followed. 2.5.3 Emergency Care Plan E. Child care personnel shall notify parents/guardians immediately of any suspected allergic reactions, as well as the ingestion of or contact with the problem allergen even if a reaction did not occur. Child care personnel shall contact 911 immediately whenever epinephrine has been administered

20-03 · Crib Requirements
In the infants (6 weeks to 6 months) room Counselor observed crib mat with rip and tear. Provider will order replacement mat. 3.6.1 Bedding and Linens Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Nap bedding is not required for school-age children; however, the program must provide an area for children that choose to rest. D. Floor mats must be at least one-inch thick and covered with an impermeable surface. 3.6.3 Crib Requirements Children up to one year of age must be in a crib or playpen/play yard with sides for napping. Crib sides must be secured while there is an infant in the crib. Bar spacing must not exceed two and three-eighths inches. Cribs or playpen/play yards must meet the regulations as outlined in Title 16, Parts 1219, 1220 & 1221 Code of Federal Regulations, 2014, which is incorporated by reference in 65C- 22.001(7)(w), F.A.C

In infant's classroom 6-12 months Counselor observed a student's medication that was expired. To comply Medicine was removed for the classroom belongs, to be sent home with parent. 6.5 Medication H.
