45-02 · Background Screening Documents
Counselor observed 2 staff members with employment dates at the facility that had more than a 90 day break in service since last being in the child care industry. Counselor gave technical assistance during inspection from the Child Care Facility Handbook: 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file Corrective Action: Provider initiated all 2 staff members resubmission of background screening during inspection. Therefore, standard was completed at time of inspection

Counselor observed the following ratio during inspection: --Mixed age group of 5 years old and School Age Children- 1 staff; 12 children (There were 6- 5 years old and 5- School Age Children) --Mixed age group of 3 and 4 years old- 1 staff; 18 children (There were 16- 3 years old and 2- 4 years old) --1 years old- 2 staff; 8 children --Mixed age group of 4 and 5 years old- 1 staff; 15 children (There were 4- 4 years old and 11- 5 years old) --Infants- 2 staff; 6 children --Mixed age group of 3 and 4 years old- 1 staff; 12 children (There were 1- 3 year old and 11- 4 years old) --1 years old- 2 staff; 11 children --2 years old- 1 staff; 11 children --2 years old- 1 staff; 10 children --5 additional staff members Counselor determined through evidence and testimony that one teacher present with between a total of 7-11 children, depending on the time, in a mixed age group of 1- 1 year old and 10- 2 years old on 6/5/2023.Counselor observed that there was one teacher present with between a total of 7-11 children, depending on the time, in a mixed age group of 2- 1 years old (child #1 and child #2) and 9- 2 years old on 6/6/2023. Counselor observed the same ratio issues on 6/7/2023, 6/8/2023 and 6/9/2023. Counselor gave technical assistance from the Florida Statutes: 402.305 Licensing standards; child care facilities. (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. Corrective Action: Provider was in compliance with ratio requirements during on-site inspection on 6/19/2023, therefore standard was completed at time of inspection

33-06 · Training Requirements
Counselor observed one staff member with employment date of 12/8/2021 that did not complete the early literacy course until 5/19/2023. Counselor gave technical assistance from the Child Care Facility Handbook: 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. One college level early literacy course (for credit or non-credit) if taken within the last five years. 4.5 Documentation of Training The Departments training transcript is the only acceptable verification of successful completion of the Departments training, early literacy courses and school-age training documentation. C. Early literacy courses and school-age training documentation must be uploaded into the Florida Pathways (Registry), which may be accessed from the Departments website at www.myflfamilies.com/childcare. Corrective Action: The staff member completed the early literacy course during inspection, therefore the standard was completed at time of inspection
