45-04 · Background Screening Documents
Licensing Specialist observed during inspection a total of 8 staff members that did not have the 5 year employment history and reference checks completed. Licensing Specialist observed the provider complete 7 staff members employment history and reference checks at the time of inspection. One staff member still needs the five year employment history and reference check completed. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 5.1 Initial Screening D. The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained. Corrective Action: Provider is to submit the one staff members employment history via e-mail to Licensing Specialist. 7 staff members employment history and reference checks were completed at the time of inspection

Licensing Specialist observed one staff member with child care industry start date of 4/17/2023. The staff member left the child care industry on 5/31/2024. The staff member became employed with the facility on 11/11/2024 and had a background screening clearance eligibility date of 4/5/2023. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 5.2 Re-Screening A screening conducted under this rule is valid for five years, at which time a re-screen must be conducted in the same manner as the initial screening. C. Child care personnel must be re-screened following a break in employment in the child care industry that exceeds 90 days. Child care personnel/individual with a break in service that exceeds 90 days are considered unscreened child care personnel/individuals until completion of re-screening. These child care personnel/individuals shall not have unsupervised contact with children in care. Corrective Action: Provider initiated a resubmission of the one staff members background screening clearance during inspection. Therefore, standard was completed at time of inspection

Licensing Specialist observed the Employee/Contractor Roster was not current or updated with the facilitys staff/personnel as of inspection date. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 5.1 Initial Screening K. The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse within 10 days of when the individual has received a child care eligible result and has been hired at the facility. Employer/owner/operator must add an end date for individuals on the Employee/Contractor Roster in the Clearinghouse within 10 days of the employment termination. Corrective Action: Licensing Specialist observed provider remove and add staff/personnel to reflect the personnel currently employed at the facility on the Employee/Contractor Roster during inspection. Therefore, standard was completed at time of inspection

Licensing Specialist determined through evidence and testimony that child #1 was left alone and unsupervised in the bathroom for less than 5 minutes on 1/23/2025. Child #1 was left alone and unsupervised in the bathroom while the 2 staff members left the classroom to go play in the facility's gym. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 2.4 Supervision Corrective Action: The 2 staff members observed that child #1 was not present upon arrival to the gym. 1 staff member left the gym, found child #1 alone and in the classrooms bathroom, and returned with child #1 to the gym. The standard was completed at time of inspection due to child #1 being directly supervised once with the 1 staff member

Licensing Specialist determined through evidence and testimony that child #1's parents did not receive and sign an incident/accident report for the incident that occurred on 1/23/2025, until 1/29/2025. LS gave technical assistance during inspection from Child Care Facility Handbook: 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. Corrective Action: The facility provided child #1's parents with documentation of the incident report on 1/29/2025, therefore the standard was completed at time of inspection

33-03 · Training Requirements
Counselor observed one staff member with child care industry start date of 5/18/2023 and employment date at the facility of 8/31/2023. As of inspection date, staff member has not begun the introductory training within 90 days of employment in the child care industry. Counselor gave technical assistance from the Child Care Facility Handbook: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel. Corrective Action: Staff member is to begin the 40 hour child care introductory training courses. Provider is to submit to counselor via e-mail the staff member's DCF training transcript showing that the staff member has begun the required training
