did not complete the required training within the time frame. 4.2.3 Safe Sleep/ Shaken Baby Syndrome Training All child care personnel who work in a facility that offers care to infants must have training regarding guidance on safe sleep practices, preventing shaken baby syndrome and abusive head trauma; recognition of signs and symptoms of shaken baby syndrome and abusive head trauma; strategies for coping with crying, fussing, or distraught child and the development and vulnerabilities of the brain in infancy in early childhood within 30 days of hire at the facility. For child care personnel to satisfy this requirement the training must be accomplished through one of following methods: Department's Health Safety and Nutrition course, Safe Sleep course, or the Early Learning Floridas Safe Sleep Practices. 39-03 · Accident/ Incident Notification and Documentation
The staff member failed to complete an incident/accident report and notify the parent of concern. The teacher admitted that she observed the child's lip and did not believe it was a big deal. Any/all incident/accidents that involve an enrolled child should be documented regardless of how little the staff member may think it is. A report was written after the mother returned to the center with the child asking what happened to her child. The report was after speaking with the mother. Even though a report was written after the fact, the counselor recommends that an all staff meeting be conducted discussing the importance of documenting all incident/accidents. A copy of the meeting notes obtaining all staff signatures. A copy of the meeting notes is to be provided to the counselor for corrective action. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur
