17-06 · Outdoor Play Area/Fencing
Counselor observed deep hole under fence with a pike sticking out of the hole. Counselor observed water fountain clonked with water. Counselor observed a work order scheduled to come out and fix playground area. 3.5 Outdoor Play Area F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. However, if outdoor play area fencing is changed then the standard would apply, and two exits must be provided. Facility has until July 03,2024 to have repairs completed

Parent went to pick up their child from their classroom and found him unattended outside. Counselor observed the transportation logs. Logs was missing driver's signature and dates. This was resolved when Driver signed and dated the Documents. Resolved at the time of inspection. 2.5.2 Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable)

Counselor observed Incident/accident reports some of parents never received or signed. This was resolved at the time of Inspection. Provider sent Incident/accident report to Counselor email. 6.3 C.
