At the time of inspection, the operator was unable to provide a documented agreement plan. 3.2 Substitute The operator must have a written plan to provide at least one substitute, 18 years of age or older, to be available on a temporary/emergency basis. A. The substitutes information (including name, date of birth, telephone number, address, anticipated number of hours worked and whether or not this person substitutes for another home) must be provided on the CF-FSP Form 5133, Application for a License to Operate a Family Day Care Home, which is incorporated by reference in 65C-20.008(1), F.A.C. B. The written plan must be kept current and include the name, address, and telephone number of the substitute . C. At the time of inspection, the operator did not provide the LS with staff files for the operator nor the substitute. Per the operator she signed the documents via the online renewal application. Despite the LS sending a renewal reminder email on 4/28/25, to the operator with an attachment requesting to have all renewal documents available upon renewal inspection, as the Attestation of Good Moral Character form is not available to sign online at the time. 4.3 Background Screening Documents B. 09-01 · Animal Vaccinations FDCH/LFCCH Handbook
At the time of inspection, the LS observed supplies for a pet at the top of the operator's stair: a sleep area/container, blankets, and a food and water source. The LS asked the operator if she had a dog her response was "no". The LS then asked about the pet supplies and if the operator had a cat in which she replied "yes". Per the operator the cat stays upstairs. (No gate was present to prevent the animal from accessing the downstairs daycare area.) Per the provider parents are aware the animal is present. 7.1 Animal Vaccinations Animals, pets or fowl must be properly vaccinated, if vaccinations are available for the type of animal, pet or fowl; and be free from disease. Animals that are poisonous, aggressive, venomous, or pose a potential threat of harm to children in care are prohibited. Custodial parents or legal guardian must be informed in writing of all animals on the premises of the home. Such information may be provided by way of a parent flyer, a notification statement, or a statement included in the child's enrollment form. Documentation of current immunizations must be available for review upon request by the licensing authority. At the time of inspection, the LS observed a child 2 years of age, sleeping in a baby bassinet. When asked if the child typically naps there the operator stated that today she just put her in the bassinet; typically, the child will sleep on a mat on the floor. LS observed the floor was not clean for a child to nap on the floor. However, due to the age and size of the child; the bassinet poses a safety hazard. 7.8 Appropriate, Safe and Sanitary Bedding A. A home must include a designated area where each child can sit quietly or lie down to rest or nap. Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Linens must be washed at least once a week and more often if soiled or dirty. All bedding and linens must be thoroughly cleaned and sanitized at least once a week and before use by another child. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. B. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs, play yards, and playpens used for infants must have tight fitting sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. D. Children must not be placed in the cribs, playpens, play yards or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. Cribs, playpens, play yards other napping and sleeping bedding being used by a child must be placed a minimum of 18 inches away from window blinds, draperies or any window treatment/cover that pose a strangulation hazard.
