    39-03 · Accident/ Incident Notification and Documentation

    The facility failed to document what occurred that day. Documentation was needed because of the severity of the allegations that occurred between two children as the teacher and the director re-directed the children for their behavior. The director took it a step further by reviewing the video footage at the center to assure that nothing occurred. The facility also did not inform the mother of the incident at pick up. An incident report was not written until the following day when the mother returned to the center questioning about something happening. The facility is to complete an all-staff meeting discussing the importance of incident/accident report writing. A copy of the meeting notes showing all staff signature in attendance is to be provided to the counselor as corrective action. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur
