(10:30 a.m.) of staff member K.S. not providing proper supervision of children due to her being on her cell phone. 2.4 Supervision 2.4.1 General Supervision Requirements A. Childcare personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the childcare facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. D. At all times, lighting must allow childcare personnel to see and supervise children while in care. E. A program is responsible for the supervision of a child until an authorized individual retrieves the child from the program. A child shall not be released to any person other than the person(s) authorized or in the manner authorized in writing by the custodial parent or legal guardians. All individuals authorized to pick up a child must be identified in writing prior to release by the custodial parent or legal guardian to the program, and the program must verify the individual picking up the child is authorized by using a picture form of identification. Each child transported must be dropped at the designated location as agreed upon by the provider and the custodial parent/legal guardian and released to an authorized individual. F. No person shall be an operator, owner, or employee of a childcare facility while using or under the influence of narcotics, alcohol, or other drugs that impair a persons ability to provide safe care and supervision. G. In addition to the number of childcare personnel required to meet the staff-to child ratio, one additional adult must attend field trips away from the childcare facility to help provide direct supervision. The individual could be a parent volunteer as long as that person is under direct and constant supervision of a screened and trained childcare personnel. H. Children must receive supervision and care in accordance with their age and required needs and be accounted for at all times while bathing or toileting. I. When transporting children in a vehicle or on foot, a telephone or other means of instant communication must be available to childcare personnel. Cellular phones, two-way radio devices, citizen band radios, and other means of instant communications are acceptable. Per Provider Staff K.S. was terminated on 6-2-25

The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Counselor was investigating a self-reported complaint involving a teacher who grabbed a one-year-old child by the arm and removed her from a table and popped her on the back of the head. The teacher then removed the child again from climbing on the table and then popped her on the bottom. As corrective action: The Teacher was immediately removed from the classroom and fired, which brings the center back into compliance for this standard and requires no additional Corrective Action

07-06 · Vehicle Requirements
Counselor observed the provider turn the vehicle on. She rode around the parking lot and parked. The alarm system never activated. Counselor waited for about 5 minutes. Per provider, they are not providing transportation services at this time. They stopped using the vehicle on 08/03/2023. They are aiming for a date of June 1st 2024 to resume transportation. Counselor gave technical assistance on Transportation requirements. They are aware that they must contact the counselor before utilizing the vehicle. Counselor will need to inspect the vehicle prior to using. The provider has given counselor a letter stating they will not utilize transportation services. This was completed and resolved at the time of inspection. 2.5.4 Vehicle Requirements For the purpose of this section, vehicles refer to those owned, operated or regularly used by the child care program, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section
