33-01 · Training Requirements
Licensing Specialist observed during inspection that 2 staff members did not complete the 40 hour Introductory Child Care Training Courses within the required timeframe. Licensing Specialist observed one staff member with child care industry start date and employment date at the facility of 8/15/2023. The staff member began the 40 hour training on 11/13/2023 and as of inspection date has not completed the 40 hour training courses. Licensing Specialist observed a second staff member with child care industry start date and employment date at the facility of 6/5/2023. The staff member began the 40 hour training on 9/1/2023 and did not complete the training until 9/4/2024. Licensing Specialist gave technical assistance during inspection from the Child Care Facility Handbook: 4.2 Training Requirements 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. Corrective Action: The one staff member is to complete the 40 hour Introductory Child Care Training Courses. Provider is to submit to licensing specialist via e-mail the staff members DCF Training Transcript showing completion of the required 40 hour training courses. The second staff member completed the required 40 hour training courses on 9/4/2024, therefore there is no corrective action due for the second staff member as the training was completed at time of inspection. Due to the upcoming holiday season, Licensing Specialist gave provider 45 days to complete the corrective action

33-06 · Training Requirements
Licensing Specialist observed one staff member with child care industry start date and employment date at the facility of 8/15/2023 that has not completed the early literacy course as of inspection date. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. One college level early literacy course (for credit or non-credit) if taken within the last five years. 4.5 Documentation of Training The Departments training transcript is the only acceptable verification of successful completion of the Departments training, early literacy courses and school-age training documentation. C. Early literacy courses and school-age training documentation must be uploaded into the Florida Pathways (Registry), which may be accessed from the Departments website at www.myflfamilies.com/childcare. Corrective Action: The one staff member is to complete the early literacy course. The provider is to submit to licensing specialist via e-mail the DCF Training Transcript showing completion of the early literacy course. Due to the upcoming holiday season, Licensing Specialist gave provider 45 days to complete the corrective action

45-02 · Background Screening Documents
Licensing Specialist observed one staff member with child care industry start date and employment date at the facility of 10/16/2024. The staff member had a background screening clearance letter with eligibility date 3/16/2023, however the staff member has never been in the child care industry until 10/16/2024. Licensing Specialist observed a second staff member that had a 5 year prints retention expiration date of 9/13/2024 for their background screening clearance letter. The provider did not have a copy of the staff members background screening clearance letter on file during inspection. Licensing Specialist was able to review the staff members current background screening clearance letter with new prints retention expiration date of 9/13/2029 via Licensing Specialists computer during inspection. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. Corrective Action: Provider is to initiate a resubmission for the one staff members background screening and submit documentation showing proof that the resubmission was conducted. Provider is to print the second staff members background screening clearance letter and submit to licensing specialist via e-mail for review. Due to DCF being closed on 11/27, 11/28, 11/29 & with background screening login issues Licensing Specialist gave provider the corrective action

Counselor observed one staff member with employment date at the facility of 6/1/2024 that had a background screening clearance letter eligibility date of 5/22/2023 on file. Counselor observed the staff member was in the child care industry from 6/2/2022 until 8/22/2022. Staff member had a 90 day break in-service from the background screening clearance letter eligibility date of 5/22/2023 and employment date at facility of 6/1/2024 and the last date being in the child care industry of 8/22/2022. Counselor gave technical assistance from the Child Care Facility Handbook: 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Counselor observed that the staff member is signed up to take the competency exams on 7/22/2023. Counselor gave technical assistance from the Child Care Facility Handbook: 4.2.1 Mandated Introductory Training Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Child care personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004 are not required to fulfill the competency examination requirement. A. 33-03 · Training Requirements
Counselor observed 3 staff member's personnel records during inspection that did not begin the introductory training within 90 days of employment in the child care industry. Counselor gave technical assistance from the Child Care Facility Handbook: 4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. The child care program is responsible for obtaining training documentation from child care personnel. Corrective Action: Counselor observed that all 3 staff members began the introductory training courses as of inspection date, therefore the standard is completed at time of inspection
