39-03 · Accident/ Incident Notification and Documentation
Licensing Specialist observed the incident/accident report for child # 1. LS observed that there was no first aid written on the incident report that was given to child #1. LS observed that there was no written documentation of the parent being notified of the incident. LS observed that the parent did not sign the incident report and that facility wrote that the parent refused to sign the incident report. Licensing Specialist did not observe an incident/accident report for the child that scratched child #1 due to an incident/accident report not being on file. Licensing Specialist gave technical assistance from the Child are Facility Handbook: 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. Corrective Action: Provider is to submit to Licensing Specialist a written statement that the facility & staff members understand and will maintain Section 6.4 from the Child Care Facility Handbook

34-04 · Credentialed Staff
Counselor observed during inspection that as of 11/21/2023 the directors credential expired, and the facility does not have a current director for the facility with credentials. The facility was placed on a provisional license starting on 11/22/2023 and will have until 5/22/2024 to obtain a credentialed director. Counselor gave technical assistance from the Child Care Facility Handbook: 4.7 Director Credential Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license. An applicant for the Director Credential must meet the requirements referenced in CF-FSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which is incorporated by reference in 65C-22.001(7)(k), F.A.C. CF-FSP Form 5290 may be obtained from the Departments website at www.myflfamilies.com/childcare. All applications and documentation will be verified, and, if complete, the credential will be issued by the Department or its designated representative on CF-FSP Form 5252, Florida Child Care and Education Program Director Credential, which is incorporated by reference in 65C-22.001(7)(g), F.A.C. Corrective Action: Provider is to obtain a credentialed director and submit documentation to counselor via e-mail of the credentialed directors personnel record

03-01 · Ratio Sufficient
Counselor observed the following ratio during inspection: Mixed age group of 1 and 2 year old- 1 staff; 7 children 2 year old- 0 staff; 9 children Infant- 2 staff; 5 children Mixed age group of 3, 4 and 5 year old- 1 staff; 16 children (There were 10- 3 year old, 5- 4 year old and 1- 5 year old. Counselor observed this ratio during naptime) 1 staff member in the office, 1 staff member in the kitchen, 1 staff member was present in their car, in the parking lot of the facility upon counselors arrival, 1 staff member arrived at approximately 2:18 PM. The 1 staff member, LY, entered the facility with counselor, 1 staff member, BK, arrived at approximately 2:18 PM. Counselor observed ratio during naptime. During inspection counselor observed, AM come from the office, into the mixed age group of 7- 1 and 2 year old children, becoming the second teacher at 2:12 PM. Counselor observed LY go to the 2 year old room. Counselor then observed staff move 1 child from the mixed age group of 1 and 2 year old to the 2 year old classroom giving the 2 year old- 1 staff; 10 children and the mixed age group of 1 and 2 year old went to just 1 year old with 1 staff; 6 children. Counselor gave technical assistance from the Florida Statutes: 402.305 Licensing standards; child care facilities. (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. Corrective Action: AM became the second teacher for the mixed age group of 1 and 2 year old. Then the provider moved 1 child from the mixed age group of 1 and 2 year old with 1 staff; 7 children to the 2 year old room during inspection. This gave the 1 staff member, 6 one year old and the 2 year old room- 1 staff; 10 children during inspection. Therefore, standard was completed at time of inspection

Counselor initially observed 0 staff member present in the 2 year old room upon arrival. Counselor observed AM present in the office, sitting at the desk. During inspection 1 staff member LY became the staff member and then, HB, became the 2 year old teacher at approximately 2:18 PM when staff member BK came from outside the facility, inside the facility and to the infant classroom. Per HB and LY, HB was in the infant room prior to 2 year old classroom. Counselor gave technical assistance from the Child Care Facility Handbook: 2.4.2 Nap time Supervision During nap time, child care personnel must be within sight and hearing of all the children. All other child care personnel required to meet the staff-to-child ratio must be within the building on the same floor and available to be summoned if needed to ensure the safety of the children. Nap time supervision does not apply to children up to 24 months of age, who must have direct supervision at all times. Corrective Action: Staff member LY and then HB became present as the teacher in the 2 year old classroom during inspection. Therefore, standard was completed at time of inspection
