Licensing Specialist observed the one-year classroom with 1 staff member and 7 children. The staff member, A.C., was observed playing with the children and had not yet notified management that she was over ratio. The director and the Licensing Specialist entered the room, greeted the teacher and spoke to her for a bit while conducting a child to teacher head count. Per the provider, the staff member usually sends a child next door to the infant room until the 9am staff member comes in on her shift. The provider took one of the children out of the room and placed them in the infant room next door, leaving the one-year-old room with 1 staff member and 6 children. The child taken to the infant room, was placed in a room with 2 staff members and 6 infants. Both classrooms were now in ratio. This was resolved at the time of inspection. The department shall establish by rule those hours of operation, such as during rest periods and transitional periods, when this subsection does not apply. (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children

B's background screening had become not eligible the day prior to the on-site inspection (6//6/2023) This standard came back into compliance by having the in individual with a not eligible background screening leave the facility until further notice. Child listed on supplemental was given over the counter medications on two occasions 01/17/2023 and 01/25/23. Both medications states for children under 2 must have a doctor's note. Child is 21 months old and there was no doctor's note on file. 6.5 MedicationChild care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care provider must never administer a medication that is prescribed for one child to another child.B. Any known allergies to medication or special restrictions must also be documented, maintained in the childs file, shared with child care personnel and posted with the childs stored medication.C. Prescription and non-prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. The facility must maintain a record for each child receiving any medications that documents the full name of the child, the name of the medication, the date and time the medication was given, the amount and dosage, and the name and signature of the person who gave the medication. This record must be initialed or signed by the program personnel who gave the medication. The record must be maintained for a minimum of 12 months after the last day the child received the dosage. G. All medication must have child resistant caps, if applicable, and must either be stored in a locked area or must be out of any childs reach. If medication is stored in the food preparation area, it must be stored in a manner to prevent contamination of food, food contact surfaces, or medication. H. Medication that has expired or that is no longer being dispensed must be returned to the custodial parent or legal guardian or discarded. I. Prior to administering medication to children, child care personnel responsible for administering medication must be educated on proper administration procedures. Written documentation must be maintained in the personnel file that child care personnel administering medication have been educated on proper administration procedures. J. Child care personnel must ensure sun safety for themselves and children under their supervision. It is recommended that infants younger than six months of age are kept out of direct sunlight, limiting sun exposure when ultraviolet rays are strongest. School-aged children attending public or non-public schools are not required to have student health examination and immunization records on file at the child care facility as such records are on file at the school where the child is enrolled. D. If the custodial parents or legal guardians need assistance concerning these requirements, the facility shall refer them to the Department of Health or to the childs physician. E. Medical records in this section are the property of the custodial parent or legal guardian and must be returned to them when the child withdraws from the facility. The medical records are transferable if the child attends another facility. F. It is recommended that child care personnel are current with all immunizations routinely recommended for adults by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC), as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by in 65C-22.001(7)(v), F.A.C

The facility loss it's Director (J.B.) on 01/06/2023. 4.7 Director Credential Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license.
