12-09 · Facility Environment
Licensing Specialist conducted a routine inspection at the facility. During the walk through of the facility, vermin was observed by the sink areas of the VPK classroom and on a table, also in the infant room by the handwashing sink. Dead bugs were observed throughout the facility. Licensing Specialist advised the owner and Director to have the classrooms cleaned up and disinfected before allowing the children to enter the areas. 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. Action Plan: The childcare personnel are going to be held responsible for the cleaning and maintenance of the classrooms. Wednesday morning a technician has been scheduled to come out and spray and add bait stations to help reduce the risk of vermin and dead bugs

21-10 · Restrooms and Bath Facilities
Licensing Specialist observed dry feces smeared on the bath facility door. This was resolved at the time of inspection. Childcare personnel wiped down the area with disinfectant cleaners. 3.7 Restrooms and Bath Facilities G. Every facility must have at least one portable or permanent bath facility available for bathing children. The bath facility must be in good operation, clean and must be sanitized or disinfected after each use. Action Plan: A checklist has been placed to remind the childcare personnel to clan and disinfect after each use

30-14 · Diapering
Licensing Specialist observed the Toddler 2's room was missing Diaper Changing procedures poster. This was resolved at the time of inspection when the childcare personnel made copies and posted them by the Diaper stations. 3.10 Sanitation L. Diaper changing procedure must be posted in the changing area and followed to protect the health and safety of children and child care personnel. Action Plan: Director will conduct a walkthrough every morning and ensure the posters are posted by the Diapering changing stations

23-05 · Fire Drills
Counselor observed the fire extinguisher was last inspected and tagged on: March 2023. Expired on March 2024. Provider called First Coast Fire and Safety Equipment and booked an appointment for this afternoon to get the fire extinguishers inspected and tagged. Corrected at the time of inspection. 3.8 Fire Safety and Emergency Preparedness To ensure the safety of children in care, facilities are required to receive yearly fire inspections by a certified fire inspector, and they must perform monthly drills/exercises to practice fire, inclement weather and lockdown procedures. Frequent practice of emergency procedures will facilitate a calm and competent reaction in response to an actual emergency, should it occur. B. Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times

Counselor observed a staff member with an industry date of 2/6/2023 who did not begun the introductory training within 90 days of employment in the child care industry. The provider has until 9/29/2023 to bring this standard back into compliance by having child care personnel start required training. Technical Assistance: Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins
