Specialist observed that one vehicle alarm went off over 1 minute. Vehicle alarm went off over 3 minutes. Alarm system is not functioning properly due to maintenance issue that was done to the vehicle when vehicle was returned. 2.5.4 Vehicle Requirements F. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Counselor and the agent determined through evidence and testimony that child #1 received a left femur fracture injury on 6/20/2024. The 2 teachers present did not observe the incident occur. The incident occurred at 4:47 PM on 6/20/2024 when child #1 fell on an object, on the floor in the classroom. The parents were never notified until child #1 was picked up at 6:37 PM. Child #1 received a fractured left femur injury on 6/20/2024 at 4:47 PM while present at the facility. The facility failed to immediately contact emergency services and the child's custodial parents or legal guardians. The parents were notified upon picking up Child #1 from the facility at 6:37 PM that there was something wrong with Child #1's leg. Counselor gave technical assistance from the Child Care Handbook: 6.3 Emergency Telephone Numbers B. For life threatening injuries, the provider shall call 911 and then notify the parent. C. Custodial parents or legal guardians must be notified immediately if the event of any serious illness, accident, injury or emergency to their child, and their specific instructions regarding action to be taken under such circumstances must be obtained and followed. 2.5.3 Emergency Care Plans E. Child care personnel shall notify parents/guardians immediately of any suspected allergic reactions, as well as the ingestion of or contact with the problem allergen even if a reaction did not occur. The counselor reviewed all 31 staff files. The counselor observed a child care personnel's transcript without the required five-hour Literacy and language course. The child care personnel's industry date is 9/20/2021, therefore she had surpassed the time frame allowed for this training to be done. Counselor observed 3child care personnel that have not begun the introductory training 90 days of employment in the child care industry.
