12-02 · Facility Environment
During the walk through on 03/17/2025 Licensing Specialist observed that the flooring in the four-year-old room under the table has tile that is chipped. Provider stated that children are picking at the floor and making it worse. Pictures taken. In the bathroom in the last class on the left the toilet is missing caps that go on the bolts to hold the toilet down. The metal toys on the playground need to be cleaned from the paint peeling and painted again. Technical Assistance given from handbook: 3.1 General Health and Safety Requirements A. All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months

13-03 · Toxic Substances, Hazardous Materials and Weapons
During the walk-through Licensing Specialist noticed on the kitchen windowsill a can of Lysol Disinfectant which sits over the dishwashing sinks. The children sit in the kitchen to eat their meals. This was corrected at time of inspection when provider moved the spray to under the locked cabinet under the sink. Technical Assistance given from handbook: 3.2 Toxic/Hazardous Material/Firearms/Weapons A. All areas and surfaces accessible to children must be free from toxic substances, bio contaminants, and hazardous materials/equipment/tools, including power tools, plastic bags, matches, candles, lighters, etc. B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times

34-05 · Credentialed Staff
Provider stated that the director only is working three hours a day every day. Licensing Specialist reminded provider that a director must be there the majority of the time that the facility is open. Corrective action: Provider must submit via email three weeks of timesheets showing that the director is there majority of the time. Technical Assistance given from Handbook: A. Each child care facility must have a credentialed director who is on-site a majority of hours, excluding weekends and evening hours that the facility is in operation. Documentation of majority of hours must be maintained and available for review by the licensing authority

07-06 · Vehicle Requirements
Counselor observed the vehicle's transportation alarm system, Doran Sleepy Child Check Monitor during inspection in the vehicle, however the vehicle's transportation alarm system was not in working condition. Counselor gave technical assistance during inspection from the Child Care Facility Handbook: 2.5.4 Vehicle Requirements For the purpose of this section, vehicles refer to those owned, operated or regularly used by the child care program, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section. F. By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. Corrective Action: Provider is to have transportation vehicle alarm system in working condition. Provider is to bring the transportation vehicle to our office for re-inspection. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. List of approved alarm manufacturers may be found on the Departments website at www.myflfamilies.com/childcare . 3. Alarms must be installed and maintained according to the manufacturers recommendations. 4. Alarm systems installed according to the manufacturers instructions prior to October 1, 2021, will be deemed to satisfy this requirement if they meet the criteria set forth in 2.5.4.F.1. above. Under this allowance, the existing alarm system must always be properly maintained in working order. Should at any time the alarm system require replacement, the new system must be selected from the Departments approved list referenced in 2.5.4 F.2. above

40-16 · Medication
Counselor observed 2 dog medications, amoxicillin/clavalunate, that were stored and present in the refrigerator on a shelf with food, such as cheese, lunch meat, lettuce and carrots. Counselor gave technical assistance from the Child Care Facility Handbook: 6.5 Medication G. All medication must have child resistant caps, if applicable, and must either be stored in a locked area or must be out of any childs reach. If medication is stored in the food preparation area, it must be stored in a manner to prevent contamination of food, food contact surfaces, or medication. Corrective Action: Provider removed both dog medications from the refrigerator and the provider removed them from the facility. The food that was in the same shelf as the medications were thrown away during inspection. Therefore, the standard was completed at time of inspection
