The facility failed to provide proof of monthly inspection log during onsite inspection. Log will need to be completed showing inspection for the month of June and provided for corrective action. 3.12 Outdoor Equipment A. A child care facility must provide and maintain enough usable equipment suitable to each childs age and development and of a quantity for each to be involved in activities. B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months

03-01 · Technical assistance was provided regarding ratio, and when to complete chores to keep the classroom adequately supervised
Specialist came into the infant classroom and observed 5 infants with 1 personnel. Staff present stated that the other teacher had stepped out to take out trash. Specialist stated to the teacher that this was not permitted, and the classroom was out of ratio. Soon after the other teacher came back into the classroom. Specialist observed the following ratio; Classroom A with 3 one-year olds to 2 personnel. Classroom B with 4 one-year old to 2 personnel. Classroom C with 6 four-year olds and 2 five-year olds to 1 staff. Classroom D with 5 three-year olds to 1 personnel. Classroom E with 11 two-year olds to 1 personnel. Classroom F with 5 infants to 1 personnel, another personnel later returned to the classroom
