    Licensing Specialist observed 7 staff members that did not have current child abuse & neglect forms on file during inspection. Licensing Specialist gave technical assistance from the Child Care Facility Handbook (Section 2.1- Health and Safety, Page 3): 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. Corrective Action: One staff member signed an updated child abuse and neglect form during inspection. The 6 staff members are to complete a current child abuse and neglect form and submit the documentation to Licensing Specialist via e- mail by the corrective action due date

    44-02 · Background Screening Documents

    Licensing Specialist observed two staff members that were missing documentation of the Level 2 Clearinghouse screening clearance. One staff member with employment date, 1/7/2025 did not have an eligible Background Screening Clearance Letter until 4/16/2025. A second staff member with employment date, 9/10/2025, had a Background Screening Prints Retention date expiration date of 1/7/2025. Licensing Specialist could not observe a current eligible Background Screening Clearance Letter for the second staff member during inspection. Provider stated the second member went on 4/25/2025 to initiate a new screening. Provider sent the staff member home during inspection. Licensing Specialist gave technical assistance from the Child Care Facility Handbook, Section 7.4.1,C (Section 2.1- Health and Safety, Page 3): 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file Corrective Action: The one staff member with employment date 1/7/2025 received an eligible Background Screening Clearance Letter on 4/16/2025. The second staff member was sent home during the inspection & will not return to the facility until the provider receives an eligible Background Screening Clearance Letter. Therefore, standard was completed at time of inspection

    Counselor observed 3 staff members all with employment date at the facility of 8/14/2023 that have not completed the school readiness training as of inspection date. Counselor gave technical assistance from the School Readiness Handbook for Public and Nonpublic Schools: 18.1 Preservice Timeframe All pre-service training requirements listed below must be completed by all program personnel, volunteers and substitutes, each as defined in the handbook, within 90 days of initial employment with any provider participating in the school readiness program. This timeframe does not start over if personnel change employment to another school readiness provider within this 90 days. Personnel who have not completed all pre-service training requirements may not be allowed any unsupervised contact with or care of children in a school readiness program. Corrective Action: The 3 staff members are to complete the school readiness training. Provider is to submit via e-mail to counselor the 3 staff members training transcript showing completion of each staff members school readiness training

    Counselor could not observe one staff member's background screening clearance letter with employment date 2/26/2024 on file during inspection. Counselor observed via computer during inspection that the one staff member has an eligibility date on their background screening clearance letter of 9/16/2023. The one staff member did not begin in the child care industry within 90 days of their background screening clearance letter. Therefore, the one staff member had a 90 day break in-service and must initiate a resubmission of their background screening. Counselor gave technical assistance from the School Readiness Handbook: 19.5 Background Screening All school readiness program providers and personnel are required to be screened according to the requirements of chapter 402, F.S.12 Initial Screening. Screening information must be documented on Form CF-FSP 5131, Background Screening and Personnel File Requirements (July 2012), which is incorporated by reference. 1. Level 2 background screening, as defined in section 435.04, F.S., is required for all personnel employed by a provider participating in the School Readiness Program pursuant to section 1002.88(1)(e), F.S., and volunteers as defined in this handbooks, and includes a national and statewide criminal records search. Corrective Action: Provider is to have the one staff member initiate a resubmission of the background screening clearance letter and submit documentation of proof that the resubmission was completed to counselor via e-mail
