04-01 · Supervision
The facility did not provide adequate supervision in preventing harm to a child. As stated in the childcare facility handbook, children must receive supervision and care in accordance with their age and required needs. The children required extra help from the provider to keep the children safe, when just 2 childcare personnel were unable to prevent a child getting harmed continuously in their care. When interviewing the staff, toileting/diapering was the most frequent time the child would do the biting. Teachers were unable to prevent others getting harmed and struggled to find ways that would prevent biting. 2.4 Supervision H. Children must receive supervision and care in accordance with their age and required needs and be accounted for at all times while bathing or toileting. This was resolved when the child was placed in a different classroom with teachers that are working closely with the family to prevent accident/incidents. A Biting policy has been made for the child and utilized at the daycare and home. The child has been doing less biting

30-09 · Diapering
Licensing Specialist had interviewed staff and was told that they did forget one day to change a diaper during the routine diaper changing times. A child did sit for too long in a soiled diaper resulting in a rash. The teachers did say it was not intentional and had thought it was changed and didn't realize they missed one until the following changing time/scheduled. 3.10.2 Diapering Requirements I. When diapers, clothing or linens that are being used become soiled or wet, they are to be changed immediately and properly disposed. This was resolved when the provider noticed the child was not changed and immediately removed the soiled diaper from the child and cleaned them up

24-13 · Food Preparation Area
Licensing Specialist observed the cook distributing meals to the classrooms and prepping meals in the kitchen without wearing a proper head covering. Licensing Specialist asked for a head cover to use in order to inspect the food preparation areas. One was not available. Provider stated they ordered some hair nets and will arrive tomorrow by Amazon delivery. 3.9 Food Preparation/Food Service 3.9.1 Food Preparation Area A food preparation area is a designated room, such as a kitchen, or a designated space in a facility not normally used or accessible to the children in daily operations for indoor play, classroom, work or nap spaces, and not included when calculating usable indoor floor space. B. Child care personnel, while working in the food preparation area, must wear proper head covering, such as a hair net or hat. To prevent contact with ready-to-eat foods, child care personnel must use clean disposable gloves, utensils, or similar items in the food preparation area. 40-04 · Medication
Licensing Specialist observed a medication brought to the center for administration, was not in its original container. The parent instructed the childcare personnel to apply at every diaper change. The usual dosage reads: "Apply liberally to affected area twice daily. See package insert for full prescribing information". This information was not available. 6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: C. Prescription and non-prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. All prescription and non-prescription medication must be dispensed according to written directions on the prescription label or printed manufacturers label and maintained at the appropriate temperature. This was resolved when the provider returned the medication back to the child's legal guardian. Action Plan: Provider will ensure all medications are checked into the front office before reaching the childcare personnel in the classrooms

34-04 · Credentialed Staff
Licensing Specialist observed the acting Director does not have a Director's Credential verified on transcript. She is in the process of getting her certificate validated. Status is currently pending. This is not resolved. 4.7 Director Credential Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Licensing Specialist reviewed the Fire Drill log. Facility failed to document fire drills utilizing the approved alarm system conducted monthly at various dates and times when children were in care for the months of: February, March, April, May and June. This was resolved at the time of inspection. Licensing Specialist observed the facility conduct a fire drill and Emergency Preparedness procedures. Fire Drill was conducted at 2:30 P.M. 3.8.4 Fire Drills A. During the facilitys license year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. 21-07 · Restrooms and Bath Facilities
Upon renewal inspection the counselor observed in the 3 year old classroom there was two 3 year old children in the bathroom by themselves with the bathroom door closed. All staff was in the back of the classroom as the restroom was located in the front of the classroom. The item was corrected at the time of inspection when one of the three teachers who was in the back of the classroom came and stood by the bathroom door to assist if needed by any of the children in the restroom. The item was brought back into compliance. 3.7 Restrooms and Bath Facilities. Children must receive supervision and assistance as required by their age and developmental needs. They must be accounted for at all times while bathing and toileting
