Counselor determined through testimony and evidence that a staff member did not comply with the facility's written disciplinary policies. The staff member utilized physical punishment, humiliating, ridiculing or degrading disciplinary action with children at the facility. Counselor gave technical assistance from the Child Care Facility Handbook: 2.8 Child Discipline B. The child care facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies. Corrective Action: Provider is to have all staff members sign the facility's disciplinary and expulsion policies, and submit to counselor via e-mail that they understand and will maintain the policies

39-04 · Accident/ Incident Notification and Documentation

Counselor observed an incident report that the facility completed on 2/23/2024 for an incident that occurred on 2/22/2024. Counselor observed that the incident report was not signed by the parent until 2/26/2024. Counselor gave technical assistance from the Child Care Facility Handbook: 6.4 Accident/Incident Notification B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. Corrective Action: The parent signed the incident report on 2/26/2024, therefore the standard was completed at time of inspection
