Counselor reviewed 4 new staff records. Missing the Background Screening Requirements Form 5131, a copy of the Background Screening and student transcripts. Provider explained all those items are available but had to be printed off by the director who was not available at the moment. Provider will submit the missing documents for new staff by due date. Counselor observed the transportation log not completed with a second signature. Counselor provided Technical Assistance regarding the importance of this requirement when transporting Children. Counselor observed items that pose a hazard to the children while in the playground: An uncoiled water hose not being used and left on the ground; An inflatable rubber playground equipment no longer in use or used for its intended purpose. Car tires used in the playground with water collected inside. Counselor discussed safety measures and to how to accomplish this on the playground. Counselor discussed the Playground Safety Log and looking out for standing water everyday before children go out to the playground. Provider will complete the necessary changes by the due date.
