23-05 · Fire Drills
Counselor observed two fire extinguishers on site who had certification of February 2023. 3.8.2 Fire Safety A. Unless statutorily exempted, all child care facilities must conform to state standards adopted by the State Fire Marshal, Chapter 69A- 36, F.A.C., Uniform Standards for Life Safety and Fire Prevention in Child Care Facilities. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. If the program is granted a fire inspection exemption by the local fire inspection office, the exemption must be documented and maintained on file at the program. B. Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times. Noncompliance was resolved at time of inspection as the provider took the individual fire extinguishers to be certified during the inspection

Prior to beginning volunteering in a child care facility, a CF-FSP 5217, Volunteer Acknowledgment which is incorporated by reference in 65C-22.001(7)(e), F.A.C., and may be obtained from the Departments website www.myflfamilies.com/childcare, must be completed and on file at the child care facility for the volunteer. 33-03 · Training Requirements
4.1 Beginning Training Child care personnel including volunteers who work 10 hours or more per month must begin training within 90 days of employment in the child care industry and successfully complete Departments training within 12 months from the date training begins. Training taken prior to employment in the child care industry does not constitute begin training. The begin training timeframes begin at the time of employment in the child care industry. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. This may be accomplished by classroom attendance in a Department-approved training course, acquiring an educational exemption from a Department-approved training course, beginning a Department-approved online child care training course, or by receiving results from a Department-approved competency examination. 04-10 · Technical assistance from Child Care Facility Handbook: 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group
A three-year-old female, who is autistic and nonverbal was able to leave the program without the knowledge of childcare workers. The child was safely brought back to the program by law enforcement. The three-year-old no longer attend the program

47-08 · Access/Child Safety
On 9/7/2023 counselor commenced complaint investigation. On this day counselor asked director/owner if she had a camara facing the playground that could have captured how the child was able to leave the playground. Owner/Director stated that there were no cameras in the back facing the playground. On 10/9/2023 the licensing office got information from another agent of the department that this facility did in fact have a camera facing the playground with footage of what had transpired on 9/5/2023, the day of the incident. Counselor called the owner to inquire about the camara footage, she stated that at the time the counselor had initiated the complaint, she had forgotten that she had a camara facing the playground area. Counselor asked owner for a copy of the video footage, and owner stated that the footage was deleted by another staff member. Owner failed to notify licensing counselor about the camera footage whenever she realized she did have it. The counselor has given the facility until 11/9/2023 to put in writing a safety plan to prevent the misinterpretation, tampering of, or alteration of information presented by the facility in the event of another investigation.
