04-01 · Technical assistance provided from CCF Handbook: 2.4 Supervision 2.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. D. At all times, lighting must allow child care personnel to see and supervise children while in care. E. A program is responsible for the supervision of a child until an authorized individual retrieves the child from the program. A child shall not be released to any person other than the person(s) authorized or in the manner authorized in writing by the custodial parent or legal guardians. All individuals authorized to pick up a child must be identified in writing prior to release by the custodial parent or legal guardian to the program, and the program must verify the individual picking up the child is authorized by using a picture form of identification. Each child transported must be dropped at the designated Child Care Facility Handbook Page | 13 location as agreed upon by the provider and the custodial parent/legal guardian and released to an authorized individual. Corrective Action: Director verified child made it to the four-year-old classroom and that child was signed in on the roster. Director advised that the mother dropped child off to the other classroom by mistake. 12-07 · Technical assistance provided from the CCF Handbook: 3 Physical Environment 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. This includes grease build-up in ovens, stoves and food equipment. Corrective Action: Provider is to contact an alarm company to evaluate and troubleshoot the existing alarm system. Licensing Specialist conducted a fire drill during inspection. The monitoring station was called, and system was put into test position to conduct fire drill. Licensing Specialist and Director made a sweep of the building and proceeded to gather attendance. Licensing Specialist observed the alarm shut off before drill was completed. Licensing Specialist observed classrooms returning inside building due to alarm sound not alarming. Director and Licensing Specialist returned inside the center to reset alarm and activate another fire drill. Licensing Specialist observed alarm system shut off before swept and children counts were acquired. Director advised Licensing Specialist that teachers know to come back in the building when alarm sound is shut off. Licensing Specialist observed the ceiling tiles wet in the hallway above the fire monitoring system. Provider advised Licensing Specialist that power went out earlier today. Maintenace personnel arrived to examine water leak and replace ceiling tiles. Director called Management to report monitoring system not working properly. Management advised Director to call alarm company to examine and troubleshoot the current system. The alarm panel was reading the system was DISARMED and wouldn't allow Director to reset. The fire alarm system is not working properly

24-17 · Technical assistance provided from the CCF Handbook: 3.9.2 Food Storage Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and in an area not used for diapering. Off-site food storage is permissible only if the site of storage is a licensed child care facility under the same ownership that includes a food preparation area that meets licensing standards. A. Food containers, such as cans, plastic containers, boxes and bags must be stored above the floor on clean surfaces protected from splash and other contamination. Counselor observed food substitutions were not noted on menu. Counselor observed children being served food items not listed on the menu. Counselor was advised by assistant director that menu was change for the week due to shortage of items. Child Care Facility Handbook states the following: 3.9.3 Food Safety H. Meal and snack menus must be planned, written, dated, and posted at the beginning of each week in an easily seen place, accessible to the parents/guardians. Any menu substitutions must be noted on the menu. A generalized menu of possible snack choices for programs that receive food donations is acceptable. All meals and snacks prepared outside of the facilitys kitchen or designated food preparation area, such as catered food, must be listed along with the source. Counselor found several diaper creams and diaper rash spray containers with no signed written authorization from parent to administer. Child Care Facility Handbook states the following: 6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care provider must never administer a medication that is prescribed for one child to another child. K. Use of diaper creams and insect repellant may only be utilized with written permission from parents/guardians. Corrective Action: Assistant director removed all medications and brought to office. Counselor reviewed all (116) children files for all children enrolled. Counselor observed an expire immunization record for child (BB). BB immunization record expired 5/8/2024. Counselor observed 3 shot record (HH, WG, SG) with incorrect expiration date. The children for these records are under two years old and record on file stated that were complete until 7th grade.
