Counselor commenced an investigation after receiving a complaint. Counselor asked to view the child's enrollment forms and documents. At this time the counselor observed documents missing information on the Accident/Incident reports and documentation. Counselor observed a few Accident/ Incident Forms that were missing the parent's signature, date and time. 45-09 · Background Screening Documents
Counselor commenced an investigation on 4/8/24 where there were allegations of a teacher hitting a child on the back. During the interview process, counselor was told that the child gets removed often from her class and placed with other children and teachers in multiple rooms. While carefully reviewing the Employee Roster-Clearing house, the facility failed to remove an employee from their roster within 5 days of the employment termination. This was resolved when the provider went into the Clearinghouse roster and put a termination date for the employee. This was completed at the time of inspection. 5 Background Screening 5.1 Initial Screening A screening must be conducted as a condition of employment. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. 46-03 · Daily Attendance
Counselor observed and reviewed the attendance record for the toddler 2 class during the inspection. The clipboard had 8 children signed in and there were only 4 children present. When the counselor asked the childcare personnel where the other 4 children were, she stated her Director told her not to sign them out. The children were going to return to the childcare personnel in the afternoon some time. Children were in other room with another childcare personnel. Counselor explained the importance of maintaining Daily rosters and documenting children's departures and arrival times. Counselor showed the childcare personnel how she could document when children leave her room and return later. Director was also given a lesson on the importance of knowing and documenting the whereabouts of children in care. This was resolved at the time of inspection. Counselor showed the staff how to document the children's arrivals and departures. 7.5 Daily Attendance B. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program. Counselor observed a child care personnel with an industry date of 6/20/2022 who did not complete the 40-hour Introductory Child Care Training. This standard was brought back into compliance by having child care personnel leave the classroom and be moved to a role of other personnel until she is able to test out. Technical Assistance from Child Care Facility Handbook: Child care personnel must successfully complete 40 hours of child care training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Counselor observed 2 staff members, (Y.C.) and (N.W.), personnel records that did not complete the required pre-service training before supervising children. (Y.C.) that had a starting industry date of: 10/29/22. Counselor had observed multiple medications in buckets and zip lock bags. Counselor asked the provider for the medication logs. Providers did not have written permission or logs for the medications. Counselor gave the medications to the facility director. The facility director gave the medication back to the child's guardian. This was resolved at the time of inspection. 6.5 Medication Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given.
