04-01 · Supervision
The teacher is recorded on video stumbling, swaying back and forth and not providing supervision to the children in care. The children appear to be holding the teacher up by both of her hands to prevent her from falling over. Even though the teacher is no longer employed the center is to conduct an all-staff meeting discussing the importance of providing adequate supervision while all children are in care. A copy of the meeting notes is to be provided to the counselor as corrective action. 2.4 Supervision. A program is responsible for the supervision of a child until an authorized individual retrieves the child from the program. A child shall not be released to any person other than the person(s) authorized or in the manner authorized in writing by the custodial parent or legal guardians. All individuals authorized to pick up a child must be identified in writing prior to release by the custodial parent or legal guardian to the program, and the program must verify the individual picking up the child is authorized by using a picture form of identification. A teacher is observed on video grabbing a child and placing in a chair roughly after the cup she was drinking out of was spilled. The facility is having all staff meeting and provide each staff member of the discipline policy. Each staff member is to read and sign the policy indicating that they understand what is written. 39-03 · Accident/ Incident Notification and Documentation
The facility did provide written statements from each teacher documenting what they saw. However, the facility failed to document or notify the parents of the said children that was mishandled by the teacher. The video footage shows two children that were mishandled by the teacher after her cup was spilled by the children were running around the classroom. The facility is to write up an incident report for the two children explaining what occurred. A copy of the signed incident report by the parents is to be provided to counselor as corrective action. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian.
