03-01 · Ratio Sufficient
Upon complaint inspection on 6/14/2024 the licensing specialist observed the following. Ratio Room #1 2's 1:5 Room#2 Infants 1:2 Room#3 Mixed age 1:9 (Three 3's, Two 4's, Two 5's & Two 7-year-olds) However, after reviewing the video tape footage on 5/28/2024 the licensing specialist observed the classroom to have the following. 1:9 (Mixed age group: One infant, One 5-year-old, Two 4's, Three 3's, Two 2's) Therefore, after reviewing the video this standard was out of ratio however the teacher(s) took the infant out of the room leaving the standard in compliance. This standard was resolved on 5/28/2024. 1:1 Infant 1:8 (Mixed group One 5-year-old, Two 4's, Three 3's, Two 2's) The Florida Handbook states the following: 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A. In groups of mixed age ranges, where children under one year of age are included, one childcare personnel must be responsible for no more than four children of any age group, at all times. B. In groups of mixed age ranges, where children one year of age but under two years of age are included, one childcare personnel must be responsible for no more than six children of any age group, at all times. Corrective action: All rooms are to be in ratio according to the Florida Statutes. The teacher(s) took infant out of the classroom of 1:9 into the infant room which brought the ratio into compliance. This action was observed on 5/28/2024 via video tape recording

30-45 · Diapering
minutes. The Florida Handbook states the following: 2.4 Supervision 2.4.1 General Supervision Requirements B. Childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Corrective Action: It was observed that the staff went into the infant room and provided care for the child; not leaving the child alone in the infant room alone and not allowing children in the childcare facility to care for the infant. This standard was resolved on 5/28/2024 and the infant nor the five- year-old child attend this daycare and there are no further safety concerns for supervision. The licensing specialist advised implementing a corrective action plan stating that all employee/staff take a refresher class on Safe Sleep Practices and Identifying and Reporting Child Abuse and Neglect so there will be no future concerns with supervision/neglect. It is suggested that all classes be completed by 9/3/2024

04-10 · Supervision
On 6/3/2024 staff/employee(s) admitted that a one-year-old child was left in a room alone without any supervision. Another agent within the department witnessed and inquired about the child in the classroom area alone without Adut/staff/employee supervision. Licensing specialist interviewed all persons involved and they all admit that a one-year-old child was left alone in a classroom. The Florida Handbook states the following: Supervision 2.4 B. Childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. *Please Note: E. A program is responsible for the supervision of a child until an authorized individual retrieves the child from the program. Corrective Action: The action that should be taken is that the one-year-old child should be supervised by a childcare personnel/employee/staff at all times and never to be left alone in a classroom by themselves. The action was resolved at the time of incident on 5/29/2024 and the one-year- old child was place in a classroom that was ratio sufficient. Therefore, this standard was placed back into compliance on 5/29/2024

39-03 · Accident/ Incident Notification and Documentation
During time of the complaint inspection on 6/3/2024 and 6/14/2024 the childcare facility did not have documentation of incident/accident report, and they had no documentation of the parents' signature. The Florida Statutes state the following procedures must be applied. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program childcare personnel and the custodial parent or legal guardian. Program childcare personnel signatures may include the director/childcare personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. E. In the event of serious injury or death, the incident must immediately be reported to the licensing authority. Corrective Action The owner/staff/employees were given TA by the Licensing Specialist on 6/3/2024 that all incident accidents must be documented although the parent were called via telephone and texted through app of the child's injury (swollen/hot to touch). This child no longer attends this daycare facility. This issue was resolved and 5/28/2024 and owner/staff/employee agreed

04-18 · Supervision
Family Service Counselor (FSC) observed an unscreened individual (DR Volunteer) with an infant. The issue was resolved due to the provider/owner returned to childcare facility and was in care of the infant. FSC provided the following information: FSC gave TA from the Florida Statutes. (2)(a) An employer may not hire, select, or otherwise allow an employee to have contact with any vulnerable person that would place the employee in a role that requires background screening until the screening process is completed and demonstrates the absence of any grounds for the denial or termination of employment. If the screening process shows any grounds for the denial or termination of employment, the employer may not hire, select, or otherwise allow the employee to have contact with any vulnerable person that would place the employee in a role that requires background screening unless the employee is granted an exemption for the disqualification by the agency as provided under s. 435.07. FSC gave TA from the Handbook states the following: Childcare personnel must be re-screened following a break in employment in the childcare industry that exceeds 90 days. Childcare personnel/individual with a break in service that exceeds 90 days are considered unscreened childcare personnel/individuals until completion of re-screening. These childcare personnel/individuals shall not have unsupervised contact with children in care. Corrective Action: The childcare facility removed the staff member from being in a classroom by herself during time of inspection. The above staff DR will not be permitted into the facility to care for children. FSC gave the above TA about unscreened personnel (at no time can an unscreened individual care for children without supervision) therefore the standard was completed at the time of inspection

20-06 · Crib Requirements
FSC observed the child sleeping on a "Boppy Pillow", as well as blankets were in the crib while the child was sleeping under the supervision of DR. FSC spoke to staff and provider and the items inside of the crib were immediately removed for the safety of the child. FSC gave TA, The Florida Handbook states the following: All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. Corrective Actions: All items in the infant's crib were removed at time of inspection and TA was given due to Safe Sleep Practices

33-01 · Training Requirements
Family Service Counselor (FSC) reviewed training requirements of (JJ & LB) two employees who have not completed the 40hr Introductory Child Care Training. The two employees will have 30 days to complete the training required 4/24/2023. The following TA was given from Section 4.2.1 of the Handbook. Section 4.2.1 Childcare personnel must successfully complete 40 hours of childcare training as evidenced by successful completion of competency examinations offered by the Department or its designated representative with a weighted score of 70 or better. Childcare personnel who successfully completed the mandatory 40-hour Introductory Child Care Training prior to January 1, 2004, are not required to fulfill the competency examination requirement

33-06 · Training Requirements
Family Service Counselor (FSC) reviewed training requirements of (JJ & LB) two employees who have not completed the 5hr Early Literacy and Language Development Course. These two employees will have 30 days to complete the training required 4/24/2023. The following TA was given from Section 4.2.2 and 4.5, C of the Florida Handbook. Section 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all childcare personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. One college level early literacy course (for credit or non-credit) if taken within the last five years. Section 4.5, C Early literacy courses and school-age training documentation must be uploaded into the Florida Pathways (Registry), which may be accessed from the Departments website at www.myflfamilies.com/childcare

FSC reviewed employee file of the Director (GK)and although they had provisional status of six months, the time frame elapsed on March 5th, 2023.
